Job Description:

The Office of Pastoral Care operates twenty-four hours a day throughout the year. Consequently,

Chaplain must be willing to accept rotation during all shifts including the night shift. When in

extenuating circumstances a Chaplain is not present at the hospital, a chaplain will be on call to

address all emergencies related to pastoral care. Thus, Chaplain at SJEH will be on call to address

matters of spiritual care. The role of the chaplain includes but not limited to:

* Conducts pastoral visits and charts pastoral assessments intervention and outcome (PAIO) of all patients within 48 hours of hospital admission, create care plans, and where appropriate, conduct and chart follow-up visits with patients
* Craft pastoral care procedures and protocols for acute, long term, and home based interdisciplinary patient care. Facilitate and submit meeting notes for the daily Pastoral

Care Report gatherings
* Provides pastoral care to team members and model EHS mission, vision, values (MVV)

for volunteers, patient families and staff through one-on-one visits, group work and participation on EHS committees
* Document assessments, care plans, and visits with patients. Generate and submit meeting

notes
* Participate in duty roster and help coordinate emergency and LOA coverage for all facilities: clinical assignments; on-call; worship; orientations; trainings and in-services; and religious rites and rituals
* Attends meetings: Pastoral Care, Clinical Pastoral Education Department, Patient Relation

and Volunteer Programs; unit department meetings; care management; performance improvement; Town Halls; and others as directed
* Participates in the Clinical Pastoral Education Process by orienting and collaborating in the

supervision of chaplain residents and trainees
* Represents the hospital to the community and community to hospital: by encouraging

utilization of EHS services, sharing news, recruiting and supporting volunteers, fostering membership in hospital and community organizations and championing the needs of medically underserved people
* Participate in programs that assess and improve clinical service delivery: by conducting mock surveys, initiating and joining performance improvement projects, and working with the Patient Relations Department
* Support the mission and personnel of EHS by modeling, teaching and inspiring commitment to EHS Core Values and Customer Service Competencies, clinical excellence, healthy stress management and receptivity to change in the worship services, passing conversations and counseling offered to patients, patient families, staff, consultants and vendors
* Support the pastoral, religious and spiritual care delivery of CPE trainees
* Work collaboratively with the Director of Pastoral Care and Education or his designee to facilitate delivery of pastoral care to all patients and families in SJEH care
