Description

StarCare Specialty Health System is in search of a qualified and compassionate Licensed Vocational Nurse (LVN) who has a passion for supporting silver/senior citizens for its Program of All Inclusive Care for the Elderly (PACE) delivered by Silver Star

Responsibilities:
  • Administers prescribed medications and assists with other aspects of medication management in partnership with the patient and family, per physician's order and in compliance with regulatory and other agency policies.
  • Follows Texas Board of nursing rules and regulations.
  • Ensures compliance with all Silver Star/PACE and Center rules, regulations, policies and procedures, and other applicable standards of care, in order to ensure services are carried out according to guidelines.
  • Drawing blood for testing at the center or in participants place of residence, obtaining vital signs and rooming participants in the clinic.
  • Administer vaccines as ordered/requested.
  • Provide patient/family information and education in order to promote the patient's health.
  • Collaborate and participate in the Interdisciplinary Team (IDT), physicians, personal care attendants and other related staff to ensure comprehensive treatment and care of program participants.
  • Complete and submits documentation, including but not limited to contact notes, Care Plans, and other related documentation following established guidelines in order to meet funding source requirements.

Essential Duties:
  • Home visits as needed to meet the need of the Silver Star Participants.
  • Perform nursing assessments, medications administration, pill pack fills, wound care, and/or other nursing related care per scope of practice at the center and in participant's place of residence including hospitals and other related facilities.

Schedule: This is a full-time/non-exempt position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

Qualifications

Qualifications:
  • Graduation from an accredited school of nursing and currently licensed in the State of Texas by the Board of Nurse Examiners as a Licensed Vocational Nurse, plus four (4) years of full-time experience as a practicing licensed vocational nurse is preferred.

AND
  • Must have at least one (1) year of experience with a frail or elderly population, current, up to date immunization status medically, and cleared of communicable diseases.
  • Prefer experience in medical clinic and home health care setting, with emphasis on interdisciplinary approach to treatment

Skills & Abilities:
  • Skill and proficiency in oral and written communication,
  • Strategic planning,
  • Operating a personal computer, with Microsoft Office Products (Word, Excel, Access, and email),
  • Knowledge and skill to operate standard office equipment, i.e., fax, phone, calculator, typewriter, copier, printer, etc.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to electronically chart in a timely manner.
  • Knowledge and skill in current health care standards and Infection control practices.
  • Ability to maintain an effective working relationship with other employees, program participants and the public.
  • Knowledge of appropriate rules, regulations, and policies and procedures.
  • Ability to perform CPR, Heimlich Maneuver, First Aid, and manage seizures as taught by Staff Development.
  • Skill and capability of accurately observing and identifying significant patient behaviors and accurately describing such events in the patient's record.

Additional Requirements:
  • Must pass a pre-hire drug screen and criminal background check.
  • Must have a Texas driver's license, Texas auto liability insurance, and be insurable under the Agency's insurance.
