* Displays expertise in medication therapy and exhibits the clinical and professional skills to positively interact with physicians, nurses, residents, students, and other health care practitioners
* Knows and follows institutional pharmacy policies and procedures and medication policies and procedures
* Knows and follows state and federal laws regarding dispensing of medications
* Accurately interprets and enters medication orders in the computer for drug administration
* Contacts the provider in a timely manner when there is a concern about a medication order as needed
* Provides drug information, drug therapy recommendations, and education to physicians, nurses, patients, pharmacy staff, pharmacy residents and students related to assigned clinical areas
* Reports adverse drug events (ADE) within assigned clinical areas
* Develops department educational/monitoring tools and in-services to promote continued growth of clinical pharmacy services and up to date practice as it pertains assigned clinical areas
* Develops and maintains policies and protocols as necessary related to assigned clinical areas
* Participates in hospital committees related to assigned clinical areas where pharmacy involvement is needed
* Reviews appropriate use of medications to include optimization of use and cost considerations
* Serves as a preceptor to pharmacy residents and students, including participation in research as it applies to assigned clinical areas
* Contributes to the continued development of the practice of the profession of pharmacy

Additional responsibilities and expectations of the Floor/ICU Pharmacist:

* Works primarily in a decentralized model providing comprehensive clinical pharmacy services including but not limited to pharmacokinetics, drug dosing consultations, and adult parenteral nutrition management
* Participates in multidisciplinary rounds daily as applicable to assigned clinical areas

Qualifications /Training:

PGY1, required

PGY2 in critical care preferred

Ability to work independently, required

Licenses/Certifications/Registrations/Education:

Baccalaureate degree or PharmD Degree from an accredited school of pharmacy required

Current, unrestricted license to practice pharmacy in the state of South Carolina
