Description

Overview: We are seeking a compassionate and dedicated Peer Specialist to join our team with a focus on providing support to individuals transitioning out of homelessness or unstable living conditions into more stable housing. As a Peer Specialist, you will leverage your own lived experiences to empower and guide individuals through their journey towards stable housing and independent living.

Responsibilities:

Peer Support:

  • Offer empathetic and nonjudgmental support to individuals navigating the transition from homelessness to transitional housing or more stable housing
  • Share personal experiences of overcoming challenges and accessing resources to inspire hope and resilience.
  • Model recovery and wellness by demonstrating positive coping mechanisms and effective communication skills.
  • Maintain adherence to the National Association of Peer Supporters (NAPS) Guidelines for Peer Supporters and Supervisors

Individualized Goal Setting and Planning:

  • Collaborate with clients to identify their strengths, needs, and goals related to housing stability and overall well-being.
  • Assist participants in developing personalized action plans to address barriers to stable housing, such as securing employment, accessing healthcare, and improving financial literacy.
  • Provide encouragement and walk alongside to help participants stay motivated and accountable for achieving their goals.

Engagement:
  • Provide ongoing one-on-one engagement to Clifford residents which support their recovery goals, including through intentional disclosures.
  • Lead wellness groups, including SUD recovery groups such as AA or NA, if appropriate.

Resource Navigation and Advocacy:

  • Connect participants with community-based resources and support services, including housing assistance programs, mental health services, substance abuse treatment, and educational opportunities.
  • Advocate to ensure they receive equitable access to essential resources and services.
  • Navigate bureaucratic processes and work to overcome systemic barriers to housing and support.

Crisis Intervention and Emotional Support:

  • Respond to crises and emergencies with calmness and empathy, providing immediate emotional support and practical assistance as needed, maintaining the Peer Value of viewing crisis as opportunity
  • Utilize de-escalation techniques and conflict resolution skills to address challenging situations in a constructive manner.
  • Collaborate with care team to ensure participants receive appropriate crisis intervention and follow-up care.

Documentation and Reporting:

  • Maintain accurate and confidential records of participant interactions, progress, and outcomes using electronic databases and case management systems.
  • Participate in team meetings, case conferences, and training to share insights, discuss best practices, and contribute to program improvement efforts.

Qualifications:

  • Lived experience with homelessness, housing instability, or related challenges is highly preferred.
  • Certification as a Peer Support Specialist, Traditional Health Worker, or willingness to obtain certification within six months of hire.
  • Strong interpersonal skills with the ability to build rapport and establish trust with diverse individuals and communities.
  • Excellent communication skills, both verbal and written, with proficiency in active listening and conflict resolution.
  • Knowledge of local resources and support services for individuals experiencing homelessness or housing insecurity.
  • Ability to work independently and collaboratively within a multidisciplinary team environment.
  • Flexibility to adapt to changing priorities and respond effectively to crises and emergencies.
  • Experience working with individuals who may be actively using and comfort working at a site where there may be active use occurring.
