Day to day as a Licensed Practical Nurse ( LPN )

As a Home Health Licensed Practical Nurse (LPN) at Cornerstone VNA, you'll be the compassionate link between patients and their health goals Collaborating closely with a Registered Nurse (RN), you'll carry out individualized care plans while providing essential education to patients, families, and caregivers, empowering them to deliver safe and effective care at home. Working as part of our dynamic interdisciplinary team, you'll strive for excellence in clinical outcomes while staying up-to-date on home care regulations and reimbursement processes.

What we're looking for in a Licensed Practical Nurse ( LPN )

To thrive as a Home Health Licensed Practical Nurse (LPN) at Cornerstone VNA, you'll need a unique blend of skills and qualifications First and foremost, a Current State of New Hampshire or Compact State licensure/registration is a must, along with a valid driver's license and reliable vehicle insurance-let's get you on the road to making a difference Strong communication skills will be essential as you educate patients and families, and a customer-centric attitude will help you build rapport with those you serve. Familiarity with home care regulations and third-party reimbursement can give you a leg up in navigating the landscape of home health care.

Adaptability and problem-solving skills are crucial for working effectively within our interdisciplinary team and executing nursing care plans seamlessly. Bring your empathy and enthusiasm, and you'll be set for success in this rewarding role

Knowledge and skills required for the position are:

* Current State of New Hampshire or Compact State licensure/registration as a LPN
* A valid driver's license in good standing
