UC Health is hiring a Full Time License Practical Nurse (LPN Meds) for Bridgeway Pointe for the third shift at Daniel Drake Center for Post-Acute Care.
The Bridgeway Pointe Nurse will support Daniel Drake's Bridgeway Pointe delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures. This unit has a total of 72 residents within our assisted living facility and has other beds for memory care. Our nurses help pass medications to these residents and provide the best care to those residents within our facility.

The availability of these on-site services allows for quick response to the complex and evolving care needs for our patients.

Job Responsibilities
The Licensed Practical Nurse (LPN Meds) is an individual who holds a current state license to practice practical nursing. LPN (Meds) will have successfully completed a Board approved basic intravenous therapy course. The LPN, under the supervision of a Registered Nurse (RN), performs duties which require a basic knowledge of nursing principles and the ability to perform technical skills in carrying out nursing procedures that are culturally based and age specific.
* Contributes recommendations to the RN regarding the interdisciplinary plan of care for each assigned patient in a collaborative effort with other health care team members within the time frame indicated by unit's standards of practice.
* Provides input to the RN regarding establishing and prioritizing a list of actual, and potential, individualized age-specific patient problems and/or needs.
* Communicates formulated plan of care, as appropriate, to all levels of associates involved in the patient's care and demonstrates ability to establish priorities of care based on patient's assessment and needs.
* Gives clear instructions to patients/family regarding treatment, and involves family/guardian in the assessment, initial treatment, and continuing care of the patient.
* Identifies and responds appropriately to different patient needs resulting from behavioral health problems, unique psychological needs or those associated with religious/cultural norms.
* Demonstrates critical thinking, accountability, flexibility, and a cooperative attitude in meeting the needs of the unit.
* Nursing process is accurately and concisely documented, including the patient's response to nursing interventions and in accordance to hospital policy and procedure.
* Acts as a resource person and mentor for new personnel, floater, and/or supplemental staff working on the unit.
* Assumes responsibility for personal professional development and contributes to the professional development of peers, colleagues, and others.
Education and Experience Requirements:
* Minimum Required: High School or GED
* Educational Certification for Practical Nursing
* Prefers an Associate's degree
* State of Ohio LPN License, BLS
* Prefers 1-2 Years of equivalent experience

* Demonstrates ongoing competency in clinical performance for age related care, growth and developmental needs, treatment protocols, and psychosocial needs of patients with primary focus on adult and geriatric patients.
* Completes an annual certification of mandatory unit specific, hospital and safety education.
