Baptist Health is looking for a Home Care Hospital Liaison to join our team in New Albany, IN

The Hospital Liaison is responsible for census development through marketing and educating potential referral sources on the benefits of Baptist Health Home Care. The Hospital Liaison serves as a liaison between the Home Health Agency and hospital case managers, physicians, patients, and families to ensure continuity of quality care from the hospital setting to the home. This position provides a visible presence and ensures appropriate utilization of home care services. The Hospital liaison provides education to the case managers and other internal referral sources regarding home health care including home infusion where appropriate.

* Demonstrates commitment to the quality, philosophy, and values of Baptist Health Home Care by maintaining established policies and procedures, organizational objectives, process improvement, and safety standards.
* Demonstrates Baptist Health standards in all interpersonal relations.
* Works in conjunction with the agency intake team.
* Incorporates regulatory and accreditation guidelines into daily activities.
* Maintains broad knowledge of and advises patient/family/physician of community resources and vendors regarding procurement of equipment and/or services necessary for patient care.
* Participates in strategic planning for census development and business development opportunities, analyzing referral trends.
* Maintain thorough understanding of home care industry trends and practices.
* Maintain thorough understanding of federal and state regulatory issues relative to inducements for referrals, anti-kickback laws, etc.
* Create brand awareness.
* Understand needs based selling approach.
* Monitors hospital hold, pending reports to track referrals.

Requirements

* Current registered nurse in the state of agency location or a Bachelor's Degree in Public Relations, Marketing, or health related field, preferred Experience
* Two years' experience in a healthcare setting.
