Responsibilities
* Provides clinical expertise in the nursing management of the patients, documentation, and care planning
* Processes start of care workflow once start of care visit has been completed
* Participates in IDG
* Reviews visits plotted on SOC or Re-cert visit and adjusts as necessary
* Maintains orders in HCHB
* Manages admissions, deaths, discharges as well as assists with these to make sure proper documentation is completed
* Assists in data collection for compliance, quality assurance, implementation of nursing policies and procedures, and staff development as assigned
* Upholds effective communication with referral sources to ensure smooth transition of admissions
Qualifications
* Associate degree in Nursing with current license
* Three years practice as a Registered Nurse
* Previous Hospice experience preferred
* Knowledge of the Hospice Philosophy of Care
* Ability to use Microsoft Word, Excel, Outlook, and other software programs
