The Mary T Family of Companies is now hiring for a part-time Home Health Aide to work in our Home Health division of care

The Home Health Aide is responsible for providing personal cares and delegated nursing services to maintain the clients' physical and emotional well-being.

LOCATION: You will serve patients in the Coon Rapids area and surrounding suburbs, including Circle Pines and East Bethel, MN.

SCHEDULE: Part-time hours (up to 25 per week), Monday - Friday daytime hours (8a-4:30p). NO WEEKENDS REQUIRED

ESSENTIAL FUNCTIONS AND BASIC JOB DUTIES

* Assist the client with personal cares as identified in the assignment sheet/client care plan.
* Cares completed may include all or some of the following: assisting with dressing, grooming and oral cares, shampooing of hair, assisting with showering/bathing, applying lotions, fingernail care and foot care and other cares as assigned.
* Assisting clients with use of the commode/urinal/toilet and with adult incontinence products as assigned.
* Perform delegated nursing services as assigned by RN Manger
* Medication reminders, blood glucose monitoring, blood pressure and catheter care.
* Complete duties accurately and timely
* Help each client to reach and maintain his/her highest level of performance and independence.
* Establish a working relationship with clients, families and related professionals while performing assigned tasks.
* Observe client and report to the RN, in a timely manner, changes in the clients' physical and emotional condition such as change in attitude, appearance, loss of appetite, weight, etc.
* Document services provided accurately and consistent with agency policies and complete all required paperwork in a timely and legible manner.
* Attend required in-service training - at least 12 hours for every 12 months of employment as well as staff meetings and other agency events.
* Performs light housekeeping duties, including vacuuming, dusting, sweeping, washing dishes, making beds, washing floors and doing laundry.
* Follows established policies and infection control procedures.
* Assumes responsibility for establishing and maintaining effective working relationships with all staff members and management.
* Follows safe patient handling procedures and established safety practices for using equipment.

BASIC QUALIFICATIONS FOR THIS POSITION

EDUCATION:

* High School Diploma or GED
* Determined competent to provide services and follow procedures designated by the RN Manager

CERTIFICATION/LICENSURE:

* Valid driver's license & current auto insurance

EXPERIENCE:

* 1-year experience in a Home Care Agency is a plus

KNOWLEDGE, SKILLS, ABILITIES:

* Ability to work independently and use good problem-solving skills
* Ability to demonstrate good judgment and observation skills
* Ability to deal tactfully and compassionately with clients, their family members and other staff to exhibit good customer service skills at all times.
* Basic computer skills.
