Description

A successful candidate will be able to:
  • Demonstrate empathy during patient encounters
  • Ability to set limits and parameters with patients
  • Build longitudinal relationships with patients, making them feel comfortable, supported and ensuring their basic needs are being met
  • Demonstrate keen analytical skills, conduct root cause analyses and identify SDoH needs

Core Competencies:
  • Educate communities, especially youth aged 15-29, about safe sexual practices, including:
    • Prevention of STDs, such as: HIV, CT, GC, Syphilis, HCV, HPV
    • Encourage routine STD testing for at-risk populations at CSHC's X-Clinic
    • Addressing systemic barriers affecting healthcare access, such as racial inequity
  • Support re-testing at CSHC X-Clinic for at-risk patients and those testing positive to STDs
  • Provide individual-level support for SDoH for at-risk youth seeking STD preventative or treatment services, including STHN

Qualifications

  • A minimum of 2 years of progressive, relevant direct patient/community collaboration
  • Associates degree or equivalent experience required
  • Experience working with LGBTQIA+, youth of color, IDU and NUSB citizens
  • Proficient in written, oral English communication
  • Ability to speak Haitian Creole or Spanish fluently preferred
  • Proven ability to work with patients from culturally, ethnically and racially diverse backgrounds
  • Excellent interpersonal skills, including integrity, good judgment, team work and diplomacy
  • Thrive in a fast-paced environment, is a self-motivated, organized
  • Willingness and capacity to take responsibility
