LPN Revisit Nurse Position Purpose:
LPN: Revisit Nurse position provides skilled nursing care to the organization's clients in accordance with the plan of care as prescribed by the physician, and assigned by the supervising registered nurse.
**Hartford region**
**Monday-Friday, 8a-5:00p, weekend/holiday rotation as required**
What You Will Do:
+ Effectively and efficiently plans visit content and schedule based on client capabilities, condition, preferences (when possible), physician orders and available resources.
+ Maintains a working knowledge of applicable Federal. State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical and professional behavior.
+ Performs specific treatments and administers medications to implement the medical and nursing care plans.
+ Applies appropriate preventive, therapeutic, and rehabilitative nursing procedures and techniques to clients.
+ Utilizes interview, observation and evaluation in assessing clients and applies nursing judgment, consistent with practice standards, in making recommendations to the primary nurse and client.
Minimum Qualifications:
+ Graduate from an accredited or state approved school of practical nursing.
+ Current licensure in the state of Connecticut.
+ Must have a minimum of one (1) year experience in an acute care setting.
+ Home healthcare experience preferred.
+ Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.
+ Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
+ Must have current Driver's license and reliable transportation to and from work site.
