Metrocare provides specialty mental health services to over 10,000 children and teens and has numerous programs for babies, children and adults with disabilities, including Early Childhood Intervention, Flourishing Families, Nurturing Parenting, Supported Employment and two specialty autism clinics in Dallas and DeSoto. In conjunction with clinical care, Metrocare's Altshuler Center for Education & Research is transforming the community mental health landscape of North Texas through workforce development focused on innovative partnerships and exceptional trauma-informed training.

GENERAL DESCRIPTION:

We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, developmental disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families and adults we serve.

The HCS/TxHmL/CLASS/General Revenue program at Metrocare Services provides case management and direct personal support to eligible individuals with developmental disabilities. Services are provided in the community and residential settings. The Licensed Vocational Nurse is responsible for coordination of medical services to individuals served by HCS Nursing. The nurse functions as a member of the Interdisciplinary Team (IDT) and must work effectively with consulting physicians, other health professionals and community providers. The LVN will promote health care services delivery with consideration of individual choice.

The LVN will participate in the person-centered planning process and related activities that promote person-centered outcomes for the consumer consistent with The Council's Outcome-Based Performance Measures and HCS Standards. General tasks include participation in staffing; medical history reviews; monthly medication reviews; medical records interpretation; physician, dentist, and pharmacist consultation and coordination; staff, parent, and individual training, and other duties as delegated by RN. Emergency situations may require the LVN to stay longer than a regular shift (e.g. if an individual needs hospitalization). The LVN generally will have 24 hours on call for his/her assigned unit.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide necessary health services for consumers as required to meet their medical needs.
  • Provide nursing input into the IDT and consultation process as part of the comprehensive program planning for individuals
  • Demonstrate ability to carry out medical orders as directed after clearly considering implications of appropriateness for the consumer, relationship to current therapy under way and integration with nursing standards.
  • Administer medication competently and safely.
  • Monitor inventory of controlled drugs.
  • Train and monitor Community Living Specialists delegating the medication administration duties for a consumer. RN implements this duty in accordance with the nurse delegation rules for HCS and BON Standards.
  • Assure the following is checked: medication labels, medication supplies and storage, and medication monitoring logs.
  • Perform nursing assessments effectively, appropriately and timely.
  • Carry out teaching protocols utilizing available resources and evaluate individual's response to education.
  • Intervene as guided by the plan of care to implement nursing actions that promote, maintain or restore physical and mental health, prevent illness and effect rehabilitation.
  • Continually collects and reviews feedback regarding outcome attainment from the individual and other services plan members.
  • Makes recommendations to the individual and appropriate support team members regarding the need for changes in identified individual goals and methods for achieving those changes.
  • Presents the views of the individual regarding his/her preferences, needs, and interests during the planning process.
  • Assists in the development and achievement of individual goals that include measurable outcomes based on individual needs, preferences, and interests.
  • Maintains confidentiality of individual information in formal as well as informal settings.
  • Work cooperatively with and respect all support team members and their contributions to the individual's wellbeing.
  • Understand legal requirements and liability of all written communication. Consistently uses individuals first language in all communication.
  • Practices crisis prevention techniques.
  • Monitors individualized health care plans.
  • Attend required in service training throughout the year.
  • Actively participates and promotes person-centered planning and service activities consistent with The Council's Outcome Based Performance Measure.
  • Carry out identified job responsibilities in accordance with the policies of Metrocare.
  • Ensures compliance with health-related standards, as required by HCS and HHSC Standards. Completes documentation as required by the IDT process, Metrocare procedures, and health-related standards with HHSC standards.
  • Performs other duties as assigned.

QUALIFICATIONS

EDUCATION AND EXPERIENCE:
  • Graduate of an accredited school of vocational nursing and licensed in the State of Texas with 0 to 1 year of experience.

DRIVING REQUIRED:Yes

MATHEMATICAL SKILLS:
  • Basic math skills required.
  • Ability to work with reports and numbers.
  • Ability to calculate moderately complex figures and amounts to accurately report activities and budgets.

REASONING ABILITY:
  • Ability to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with only occasional or no variables.

COMPUTER SKILLS:
  • Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).
  • Ability to utilize the Internet for resources.

CERTIFICATIONS, LICENSES, REGISTRATIONS:
  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain a Texas Driver License within three (3) months of employment.
  • Liability insurance is required if employees operate personal vehicles on Center property or for Center business. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must have an acceptable driving record.
