  • Address immediate medical needs of clients (phone or in person)
  • Maintain confidentiality of client information at all times
  • Complete documentation and submit to RN weekly
  • Administer medications to clients as prescribed and documented on MAR
  • Complete documentation (MAR, client specific monitoring) daily
  • Participate in client's IDT (when necessary) and report any perceived areas of concern or accomplishments
  • Complete incident reports for all unusual incidents involving the client and immediately notify RN of occurrence
  • Represent the agency and client in a positive manner
  • Demonstrate understanding of agency policies and procedures
  • Serve as a liaison between SBH and other agencies, community members, and medical professionals
  • Work with physicians and specialists to complete plan of care
  • Assist in obtaining informed consent for medications/treatment
  • Assist in ensuring physician's orders are current and correct, taking off physician's orders
  • Facilitate and attend (when necessary for completion of plan of care) medical appointments
  • Routine monitoring of client specific issues (seizures, bowel habits, blood pressure, etc.)
  • Other client specific duties as assigned by the RN
