The Department of Medicine, Division of Infectious Diseases has an exciting opportunity for an Outreach Coordinator. The Outreach Coordinator oversees the development and implementation of public education and outreach programs, and assumes responsibility for creating, planning, preparing, and executing varied programs to reach targeted populations.

The goal of this position is to enhance a patient centered approach to HIV care by supporting patients with all aspects of continuous engagement in clinical care. This employee will work in coordination with the patients' medical team, case managers, scheduling office, central insurance verification, insurance companies to ensure the patients remain engaged in their comprehensive care. Identify issues that may prevent a patient from being able to attend their medical appointment and coordinate with the team to eliminate those barriers. Maintain close contact with patients newly engaged in care or who are inconsistent with their care. Assist with patient flow to ensure that patients have the best experience possible on their day of the clinic visit. Be available to assist the patient throughout any stage in their care to make sure their questions or concerns are addressed. Provide patient education as necessary, acknowledging patients' needs and values. Advocate for appropriate services for clients based on needs and assessments. Conduct visits with clients in various settings, including in the community, client's home and attending client appointments. Ability to work effectively with humans of diverse gender identities, races, ethnicities, ages, sexual orientations, and drug use patterns, in a multicultural environment. This is a critical component to the agency and the work that is done in the community. Willingness to work non-traditional hours.

Minimum Requirements:

Bachelor's degree in relevant field

Minimum 1 year of public outreach experience, syringe service program, and knowledge of HIV outpatient care

Bilingual preferred (English/ Spanish or English/Haitian creole).

Any appropriate combination of relevant education, experience and/or certifications may be considered.

Knowledge, Skills and Attitudes:

* Skill in completing assignments accurately and with attention to detail.
* Ability to analyze, organize and prioritize work under pressure while meeting deadlines.
* Ability to process and handle confidential information with discretion.
* Bilingual in English and Spanish preferred
* Ability to work evenings, nights and weekends as necessary.
* Commitment to the University's corevalues.
* Ability to work independently and/or in a collaborative environment.
