Description

StarCare Specialty Health System is in search of Habilitation Coordinators to fill an essential role in the Pre-admission Screening and Resident Review (PASRR) process.

Responsibilities of PASRR Habilitation Coordinators Include:
  • Using the discovery process to develop the Individual Plan (IP) and Habilitation Service Plan (HSP).
  • Ensure all Specialized Services are implemented in a timely manner.
  • Conduct monthly face-to-face visits with individuals in nursing facilities.
  • Conduct semi-annual Community Living Options (CLO).
  • Perform on-going monitoring activities.
  • Be responsible for scheduling and facilitating the service planning team (SPT) meetings at least quarterly and revise HSP as needed.
  • Assist with completing PASRR Evaluations (PE) when needed and when time permits.
  • Communicate information to supervisor and other employees as needed.

Schedule:

Full-time (40 hours per week) non-exempt position scheduled Monday - Friday, 8am - 5pm with flexibility to meet the needs of the program.

Qualifications

PASRR Habilitation Coordinator Qualifications:
  • Bachelor's degree from an accredited four (4) year college or university with major in a Human Services or related field (Sociology, Psychology, Family Studies, etc.), and one (1) year of full-time experience working with individuals with Intellectual or Developmental Disabilities (IDD) is required.
  • Experience working with individuals with Mental Illness is preferred.

Additional Qualifications:
  • Experience with Pre-Admission Screening and Resident Review (PASRR) is preferred.
  • Bilingual (English/Spanish) also preferred.
  • Experience working with Nursing Facilities, preferred.
  • Knowledge and skill in operating a personal computer with Microsoft Office products is required.
  • Must be able to communicate effectively, both orally and in writing.
  • Must be able organize and prioritize a variety of assignments and manage time effectively.
  • Must be able maintain effective working relationship with clients, other employees, and the public.
  • Must have current Texas driver's license and be insurable under Center insurance.
  • Must pass pre-hire drug screen and criminal background check.
