**Description:**

**Position Purpose**

CHHA provides personal care and promotes maintenance and rehabilitation if applicable of the patient under the supervision of a Registered Nurse and in accordance with a plan of treatment. CHHA facilitates a safe home environment. Must have knowledge and skills necessary to provide patient care that is age specific to the patients served. Ages served range from infant to adult.

**What You Will Do**

+ Provides overall personal care to patients following the written plan of care. Allows for patient input and preference in this plan.

+ Assists patients with or provides patients with personal care as directed. Ensuring safety of patient.

+ Assists patients in ambulating as directed by nurse. Performs simple procedures as an extension of therapeutic services including P.R.O.M. and A.R.O.M.

+ Transfers patients from bed to chair/wheelchair and back using proper lifting and transferring techniques and/or equipment. (Gait belt, Hoyer lift, bench, walker, cane, wheelchair.)

+ Provides simple meal preparation, linen change, personal laundry, tidying bedroom, tidying bathroom, dishwashing, and light housekeeping as directed.

**Minimum Qualifications**

+ Current Certified Home Health Aide - Iowa Registry required.

+ Expected to take weekend calls, holiday calls, and make home visits as required.

+ Must apply and obtain Illinois licensure.

+ Must provide current, valid driver's license.

+ Must provide current, valid auto insurance.

+ One-year health aide experience required; Home Care/Hospice experience preferred.

+ High school graduate or equivalent.

+ Must be BLS certified within 3 months of hire.

+ Must possess good oral, written, communication, and observational skills.

+ Must be able to complete work efficiently, effectively and independently.

+ Hospice training if applicable.

+ Must be comfortable operating in a collaborative, shared leadership environment.

+ Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission and vision, goals, and values of Trinity Health.
