Description

Western Wisconsin Health is seeking a full-time Medication Therapy Management (MTM) Pharmacist (.8 FTE) to join our team. Under the direction of the Pharmacy Director, implements and oversees all pharmacy services within Western Wisconsin Health. The MTM Pharmacist ensures proper pharmaceutical care and positive patient outcomes in the hospital and clinic setting. Maintains an environment of collaboration and cooperation with all departments and with other colleagues. All of this is done to support the organization's mission while living our values.

QUALIFICATIONS

Minimum Qualifications:
  • Master's degree in Pharmacy, Rph, or PharmD
  • Wisconsin Pharmacy License
  • 1+ years MTM pharmacy experience or completed residency

Preferred Qualifications:
  • Board Certified Medication Therapy Management Specialist ( BCMTMS)

Knowledge, Skills and Abilities:
  • Maintain the integrity of highly confidential business and patient information
  • Display and promote high standards of ethical conduct and behaviors consistent with organizational standards.
  • Working knowledge of Microsoft Office (Excel, Word, PowerPoint, Outlook) and electronic medical records.
  • Exhibits excellent verbal and written communication skills.
  • Exhibits excellent organization and time management skills.
  • Knowledge of pharmacy distribution systems and clinic opportunities.
  • Ability to adapt to varied situations requiring prioritization, constructive thinking, analysis, delegation and coordination and evaluation.
  • Ability to anticipate, understand, and manage patient expectations and provide consistent delivery of services

Work Schedule:
  • .8 FTE - 64 hours per bi-weekly pay period
  • Monday - Friday during clinic hours
