About Your Work

The Supportive Care LVN is responsible for partnering with the interdisciplinary team to provide best-in-class quality of care to our members. Responsible for developing strong therapeutic relationships with patients, their care partners and the healthcare teams that support them. This individual utilizes their outstanding clinical skills to implement care plans that align with the needs and preferences of the patient. Understands and helps to educate patients about advanced care planning in collaboration with the Social Worker and interdisciplinary team. Responsibilities can include obtaining and documenting vitals, collecting laboratory specimens, recording care data in a timely manner, and assisting providers and RNs with patient care support activities. This position requires home visits as part of comprehensive care of our complex patients.

You Will Focus On:
  • Serve as the primary point of contact for patients and their families enrolled in the supportive and palliative care program.
  • Implement plan of care initiated by registered Nurse and IDT team.
  • Elicit and help maintain the type of care preferred by the patient.
  • Provide emotional and logistical support for patients and their families to help them make informed decisions.
  • Facilitate timely communication between the healthcare team, patients, and their families.
  • Coordinate healthcare resources and services to optimize patient care.
  • Work collaboratively with an interdisciplinary team.
  • Conduct home visits.
  • Educate patients and caregivers on medications, symptom management, and available services.
  • Provide information on advanced care planning and facilitate discussions if necessary.
  • Actively participate in weekly interdisciplinary care rounds.
  • Work closely with external agencies and organizations to ensure continuity of care.
  • Ability to present educational materials to all levels of learners; and responds to questions among a variety of audiences.
  • Ability to adapt and respond to stressful situations in a rapidly changing environment.
  • Effective verbal and written communication skills, to represent the organization and be a patient advocate; managing both internal and external stakeholders.
  • Participation in professional and personal growth and development opportunities.

The Skills, Experience & Education You Bring
  • Graduate of an accredited school of vocational/practical nursing.
  • Active California LVN License.
  • Certification in Hospice and Palliative Vocational/Practical Nursing, preferred but not required.
  • 1 year of experience in a clinical/healthcare environment with demonstrated excellent clinical care and communication skills.
  • Coordination of Care Experience.
  • Epic experience preferred.

Final determination of a successful candidate's starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. We provide a competitive compensation package that recognizes your experience, credentials, and education alongside a robust benefits program to meet your needs.

The anticipated pay range for this role is listed in our salary posting for transparency but may vary based on factors including the candidate's qualifications, skills, experience, and geographic location. This base pay range is specific to California and can vary based on Bay Area, Metro LA, and Greater California regions which may not be applicable to other locations.

Physical Requirements: Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork - Activity level: Sedentary, frequency most of workday.
