Position Summary

U n de r m i n i mal s u pe r v i s io n and using clinical experience, evidence-based knowledge and in collaboration with our physicians, to process clinically appropriate care management referrals.

Essential Functions and Responsibilities of the Job

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications.
