Case Management Intern

A Case Management Intern will gainpractical experience in the field of social work, human services, or a related discipline. Under the supervision of experienced case managers, the intern contributes to providing holistic support and coordination of services to clients facing various challenges. This internship is designed to offer hands-on exposure to case management processes, resource navigation, and client advocacy.. This internship is designed for BSW candidates or students pursuing Human Service Degree.This is an unpaid internship opportunity.

Responsibilities:

* Conduct initial client assessments to identify needs, strengths, and goals.
* Collaborate with case management team to gather relevant client information.
* Assist in the development of client-centered case plans based on assessment findings.
* Collaborate with clients to set realistic and achievable goals.
* Connect clients with appropriate community resources, social services, and support networks.
* Provide guidance on navigating available resources and programs.
* Coordinate with healthcare providers, social workers, and other professionals to ensure comprehensive care.
* Facilitate communication and collaboration among different service providers.
* Regularly review and assess clients' progress toward established goals.
* Adjust case plans as needed based on changes in clients' circumstances.
* Maintain accurate and up-to-date client records, documenting interactions, referrals, and progress.
* Ensure compliance with confidentiality and legal standards in record-keeping.

Qualifications:

* Enrolled in a Bachelor of Social Work program or pursuing Human Service degree
* Strong interpersonal and communication skills.
* Ability to work collaboratively in a team setting.
* Understanding of and commitment to ethical social work practices.
* Demonstrated interest in and commitment to mental health and therapy services.
