Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY: The Behavioral Health Manager provides brief interventions and therapy integrated with other health providers, including Primary Care, Perinatal, Dental and other behavioral health and substance use providers. EDUCATION, EXPERIENCE, TRAINING
  1. LCSW/LMFT/LPCC (California BBS licensure is required).
  2. A minimum of two-years experience working in behavioral health / mental health setting providing diagnostic, crisis, and assessment services.
  3. Experience with program development, outcome reporting, and leading teams.
  4. Professional history of providing training and presentations preferred.
  5. Must provide proof of current driver's license and auto insurance, ability to travel in Yolo County (West Sacramento, Davis, Woodland). Travel between sites required.
  6. Previous experience with Avatar EHR strongly preferred.
  7. Bilingual Spanish/English preferred.


KNOWLEDGE OF WORK

1.

Direct clinical experience providing services to those living with Severe Mental Illness (SMI) and/or Substance Use Disorders.

2.

Knowledge of Trauma-Informed principles and practices.

3.

Ability to work well within a multidisciplinary team including law enforcement, city and county officials, and medical staff

4.

Must have a commitment to excellence and dedication to serving at-risk and vulnerable populations including those experiencing any or all of the following: homelessness, mental illness, substance use disorders, cognitive delays, legal involvement, and extreme poverty

5.

Understand and accurately use DSM 5 and ICD10.

DUTIES AND RESPONSIBILITIES

1.

Provide administrative oversight, development, and evaluation of all assigned programs.

2.

Positively contribute to Behavioral Health Programs and Management Team, support fellow managers in the functions of their jobs, provide coverage when needed, attend management meetings, support/facilitate departmental meetings and training opportunities, work collaboratively to problem solve departmental issues and challenges, support with staff recognition and engagement efforts.

3.

Maintain responsibility for recruiting, training, developing and evaluating staff (including clinicians, case managers and peer advocates).

4.

Utilize principles of anti-racism and cultural humility, fosters a culture of inclusion and belonging, and facilitates and supports staff involvement in DEI activities.

5.

Participate in and directly observe clinical and case management delivery of services through regular and frequent interface with program staff and clients. In rare instances, may include responding to after-hours emergencies for programs providing on-call coverage.

6.

Respond to crisis, triage client needs, and conduct comprehensive biopsychosocial assessments for those experiencing moderate to severe mental health symptoms and conditions, provide care coordination and linkage, understand billing structure, and how to correctly utilize and interpret ICD10 and DSM5 codes and criteria.

7.

Utilize ASAM screenings with clients seeking Substance Use Disorders Treatment Services.

8.

Communicate proactively with Clinical Director, support with problem solving and program development efforts, and serve as role model for professional and mission driven conduct.

9.

Represent CommuniCare+OLE and programs at community events, meetings, and workgroups via formal presentations and informal collaborations.

10.

Collaborate directly with program case managers, clinicians, peer advocates, partner agencies, and community stakeholders to advocate, support program outcomes, and advance client care.

11.

Monitors/completes clinical documentation to ensure the timely placement of all correspondence, completion of all screening and data collection tools, linkages to care, and accurate completion of administrative functions according to protocol and best practices. Participate in quality assurance and improvement efforts.

12.

Maintain adherence to all policies regarding confidentiality, HIPAA, 42CFR Part 2, mandated reporting and Tarasoff.

13.

Responsible for understanding, explaining and enforcing all rules and regulations associated with clinic policy.

14.

Compile, analyze, and report on outcome data as required for all programs. Analyze and adjust processes as needed. Make data-driven decisions.

15.

Attend and/or facilitate clinical consultation, staff meetings, meetings with community partners, training as arranged/required.

16.

Maintains current license/registration in good standing, submits all CEU and training certifications to HR.

17.

Responsible for maintaining the standards as listed in the CommuniCare+OLE Policies and Procedures, and any other standards which may apply to this position

18.

May require direct transportation (in personal vehicle) of clients when program vehicle is not available and as deemed safe to do so.
