POSITION SUMMARY

Carries out assigned responsibilities of the Infectious Disease Prevention Division. Duties include identifying, locating, notifying and providing referrals for treatment and examination for individuals who have been diagnosed with or in close contact with a communicable disease. Contact tracing, outbreak investigation, and continued monitoring of individuals may be necessary depending on diagnosis.

SUPERVISION EXERCISED

Provides no direct supervision to Health Department staff.

SUPERVISION RECEIVED

This position is under the direct supervision of the Communicable Disease Nurse Manager.

ESSENTIAL FUNCTIONS:

* Locate individuals diagnosed with communicable diseases, ensure diagnosis has been provided, deliver comprehensive education, treatment and prevention options in accordance with local, state, and federal guidelines.
* Interview individuals diagnosed with communicable disease to identify associated partners/close contacts, ascertain disease origin, if possible, and identify outbreaks or clusters.
* Provide partner/close contact notification, testing, referral, and education where applicable via direct service or referral.
* Ensure mitigation strategies are effectively delivered to patients, partners/close contacts, agencies, businesses, medical providers, etc., where applicable.
* Examine medical records, communicate with medical providers, interview partners and close contacts, and review information provided through electronic databases to ensure a comprehensive understanding of the patient and the disease, cluster, and/or outbreak.
* Develop and maintain detailed records of all activities, events, and information obtained.
* Communicate effectively by demonstrating professionalism, establishing rapport, listening effectively, providing information, and soliciting patient feedback.

Job Requirements

JOB REQUIREMENTS AND QUALIFICATIONS

* Requires an Associate Degree from an accredited university and two (2) years of equivalent experience in infectious disease prevention.

- OR -

* A High School Diploma and ten (10) years of experience in infectious disease prevention.

A working knowledge of Excel, Provide Enterprise, I-NEDSS, REDCap, I-CARE, Salesforce and/or other IDPH recommended platforms is preferred.

ESSENTIAL PHYSICAL/COGNITIVE ABILITIES

* The ability to sit, flex, stand, reach, and bend.
* The physical strength to lift, carry, and transport materials/equipment equivalent to thirty (30) pounds.
* Visual acuity, auditory ability, and clear written and verbal communication ability.
* Manual dexterity and the ability for data input into various computer programs.
* The ability to maintain professional behavior at all times.

SPECIAL REQUIREMENTS

* Must reside within St. Clair County, IL or be willing to relocate to St. Clair County within six (6) months from hire date.
* The ability to work necessary hours to meet the position requirements including evenings and/or weekends as assigned.
* May require provision of services at off-site locations.
* A valid Driver's License.
* A dependable automobile for use in position.
* Auto insurance in effect with at least the minimum limits of liability as outlined and required by the state of Illinois.
* Adherence to all local, state and federal regulations pertaining to OSHA guidelines.
* Health Department employees:
* Must be current on CDC Healthcare Personnel Vaccine Recommendations, which include Hepatitis B, MMR (Measles, Mumps Rubella), Td/Tdap (Tetanus/Diphtheria/Pertussis), Varicella (chickenpox) and Meningococcal if over 65. Influenza and COVID vaccines are recommended and encouraged, but not required at this time and maybe subject to change.
* Are required to have a TB test upon hire and separation of employment.


* All employees must adhere to the St. Clair County Personnel Code.
* Position contingent upon continued grant funding.
