In the role of a Qualified Intellectual Disability Professional at Pathfinder, your primary focus is on coordinating and overseeing Individual Support Plans (ISP) for individuals with intellectual and developmental disabilities in group home settings. Embracing a person-centered thinking model, you maintain positive relationships and encourage its use within the team and among individuals supported.

Your responsibilities extend to creating Behavior Support Plans (BSP) when needed, ensuring HRC approval, and providing guidance to Direct Support Professionals (DSPs). By conducting individual assessments, you tailor objectives and behavior plans within the ISP framework. Your leadership involves providing training to DSPs and individuals supported in areas like informed choice, person-centered planning, quality of life, and empowerment.

You actively participate in person-centered planning efforts, facilitating discussions, arranging necessary services, and maintaining group home files. Screening potential individuals for group home placements, assisting in medical appointments, and ensuring compliance with regulations are integral parts of your role. Balancing flexibility and productivity, you spend approximately five hours per week on direct care in each assigned home, coordinating with home managers.

Your commitment to ongoing professional development is evident through attendance at required trainings. Acting as a primary contact with IDT members, collaborating with Group Home Managers, and ensuring effective communication flow contribute to the success of your role. Additionally, you cooperate with auditors, addressing findings promptly and implementing corrective action plans.

Requirements

  • Bachelor's degree required in a developmental human service or therapy field.
  • Minimum of two years of experience in the Human Service field.
