Job Description

This position exists to provide comprehensive medication-related care to achieving definite outcomes that improve patient's quality of life. The pharmacist acts in collaboration with physicians and other health care professionals in the responsibility for patient drug therapy outcomes. To carry out the functions and activities of the Pharmacy Services department to provide the pharmacy services for patients and other individuals and groups. This is a PRN position that would cover 1st, 2nd, and 3rd shifts as needed.

Education:

* Graduate from a School of Pharmacy. Doctor of Pharmacy preferred.

Experience:

* Experience in hospital pharmacy with at least 1 years' experience in a relevant Clinical/ Pharmacokinetic Dosing/Rounding role preferred.
* Training in IV preparation and aseptic technique preferred.

Licensure & Certification:

* Requires current Indiana Pharmacist license

General Responsibilities

* Verbal ability is needed to interpret and understand written physician orders, formularies, and reference texts, as well as for presenting data information on pharmaceutical to physician, staff, and public.
* Must understand the composition and properties of drugs and their pharmacologic actions.
* Numerical ability is needed in measuring, weighing, and packaging drugs, in computing charges for preparation and in record keeping.
* Form perception is needed to interpret prescription format, to note physical or chemical changes in ingredients for prescriptions, and to interpret formularies.
* Clerical perception is needed to prevent errors in reading formulas and in proofreading physician orders for patients. It is vital also in report and record keeping.
* Finger dexterity is needed in handling small quantities of compounds, in mixing IV's, filling capsules, and other fine tune skills.
* Professionalism: Uses good problem-solving skills; demonstrates the ability to assess a situation, research additional information if needed, and make an appropriate, informed decision or recommendation; shows confidence in making decisions; consults with the clinical pharmacy specialists and/or pharmacy manager when appropriate. Demonstrates good judgment and decision making skills. Requires verbal and written communication skills. Requires ability to prioritize. Requires ability to be involved in multiple tasks/projects. Requires ability to work with others. Requires close attention to detail. Reads electronic mail in a timely manner.
* Medication Infusion Knowledge: Prepares IV admixtures and extemporaneous parenteral products under aseptic conditions; including chemotherapy and other hazardous medications as needed. Possesses understanding of USP 797 guidelines and assists in its compliance.
* Patient Medication Management: Verification of provider-entered orders for appropriate drug, dose, route, frequency and indication. Performs patient profile review for allergies, drug-drug interactions, therapeutic duplications, drug-disease interactions and drug-food interactions. Confers with practitioner in a timely manner for any questions or irregularities regarding medication orders. Documents all interventions in the Cerner system or other clinical surveillance tool. Remains current with automation technology (e. g. , dispensing systems, drug information resources, etc). Competent in performing computer functions. Provides formulary alternative agents to prescribers as required for inpatient medication therapy management. Continually works to ensure patient safety as it relates to medication therapy. Ensures proper packaging and labeling of medications before dispensing.
* Initiative and Adaptability: Uses manpower effectively to ensure medications are dispensed in an accurate and timely manner. Adapts to changes in acuity and effectively manages workflow. Readily offers assistance to peers and remains flexible during staffing changes. Maintains medication order verification, patient education, adverse drug event reporting, intervention documentation, and clinical functionalities as benchmarked against pharmacist peers with similar shift assignments. Identifies, implements, and monitors at least one quality and/or workflow improvement initiative.

Supervision: Coordinates the workflow of the department so that medications are provided in an accurate and timely manner. Directly oversees the work performance of ancillary personnel (pharmacy technicians and purchaser) to ensure adherence to established standards, regulations, and laws in medication preparation and distribution. Also supervises pharmacy technicians, vendors, and/or students on rotations from an ACPE accredited pharmacy school. Requires little supervision to carry out daily operational responsibilities. Serve as Pharmacist in staffing at Starke Hospital or other system capacity as part of a rotation or as needed and assist Starke Hospital and surrounding system with workflow completion from La Porte Hospital as requested.

* Education: Provides medication education to other health care providers, patients, and family members as needed. Perform patient bedside medication education and counseling. Participates in various educational opportunities. Remains compliant in license-required CE courses. Participates and completes all departmental and institutional competencies as may be required or assigned. Has knowledge of federal and state laws and regulations pertaining to dispensing of medications and controlled substances.

Exhibits a professional attitude when dealing with patients, colleagues, physicians and visitors. Demonstrates good rapport and teamwork with all pharmacy staff and other disciplines. Contributes positively to colleague and departmental morale. Abstains from contributing to hear-say or participating in unprofessional conversations that negatively impact the work environment.
* Inventory Management: Assists the purchaser with monitoring and maintenance of inventory and stocking levels in the pharmacy. Helps ensure adequate medication inventory is stored on the nursing units. Helps to insure pharmaceuticals are kept under proper conditions of storage and security. Computes, corrects, records, and processes charges for entry on the computer system(s).
* Drug Information: Serves as a source of drug information for nursing, physicians and other healthcare professionals including IV compatibility, appropriate drug selection, appropriate dosing, frequency, medication indication, administration guidelines, contraindications, and cautions. Provides information to physicians and other staff members on availability of new drugs, warnings on currently marketed drugs, incompatibility of certain drugs, manufacturer backorders and any other pertinent pharmaceutical information.
* Quality: Participates in all department quality initiatives as well as clinical programming as established by the department's Director. Participates in medication use evaluation projects and other projects as determined by pharmacy leadership team. Able to complete or assist in medication utilization evaluations and any other, pharmacy-driven quality projects.
* Clinical: Appropriate application of Antibiotic Stewardship Principles according to IDSA and best practice guidelines. Has command of dosing of all medications as it pertains to four age-related groups: neonatal, pediatrics, adult, and geriatric patients. Pharmacists will be pro
