We are seeking and actively recruiting skilled and qualified Pharmacists as candidates for 2 full-time Direct Hire (Day shift) positions. The positions are located at a great hospital in the Fort Wayne area. We have one position available for a clinical generalist and one position available for a Pediatrics/NICU Specialist.Candidates for both roles must possess a high level of competency/experience, a pharmacy degree from an ACPE-accredited institution, completed required Pharmacy residencies, and have an active, Indiana Pharmacy License. The pay rate is based on experience and has a range of $51.94-77.91 per hour. The Direct hire would also be eligible for full-time benefits, with negotiable salaries and sign-on bonuses. There is also the possibility of a relocation bonus.

Clinical Generalist Pharmacist Responsibilities:

* Review medication orders, assess appropriateness, safety, and efficacy of prescribed medications, and make recommendations for therapy optimization based on patient-specific factors such as diagnosis, comorbidities, and laboratory values.
* Provide evidence-based pharmacotherapy consultation to healthcare providers, including physicians, nurses, and other allied healthcare professionals, regarding medication selection, dosing, administration, and monitoring.
* Research and provide accurate drug information to healthcare professionals, patients, and caregivers regarding medications, including indications, contraindications, adverse effects, drug interactions, and alternative treatment options.
* Participate in multidisciplinary rounds and patient care conferences to collaborate with other healthcare team members in developing and implementing individualized patient care plans, including medication therapy management.
* Provide medication counseling and education to patients and caregivers on medication use, dosage instructions, potential side effects, drug interactions, and adherence strategies to promote safe and effective medication therapy.
* Conduct medication reconciliation upon patient admission, transfer, and discharge to ensure accurate and comprehensive medication lists, reconcile discrepancies, and prevent medication errors and adverse drug events.
* Document clinical interventions, medication-related assessments, recommendations, and patient outcomes in the electronic health record (EHR) or other documentation systems in accordance with institutional policies and regulatory requirements.
* Implement medication safety practices and protocols, such as high-alert medication monitoring, medication error reporting, and participation in medication safety initiatives and committees, to minimize the risk of medication-related harm.
* Participate in antimicrobial stewardship activities, including antibiotic utilization review, infectious disease consultations, and development of antimicrobial guidelines and protocols, to optimize antimicrobial therapy and combat antimicrobial resistance.
* Participate in quality assurance activities, medication use evaluations, performance improvement projects, and regulatory compliance audits to monitor and improve medication use processes and patient outcomes.
* Collaborate with the pharmacy and therapeutics (P&T) committee and other stakeholders in the development, implementation, and management of the hospital formulary, including formulary reviews, drug evaluations, and therapeutic interchange programs.
* Stay current with advances in pharmacotherapy, evidence-based medicine, and healthcare regulations through continuing education, participation in professional organizations, and self-directed learning activities.

Pediatric/NICU Pharmacist Responsibilities:

* Review medication orders for pediatric and neonatal patients to ensure accuracy, appropriateness, and safety, taking into account factors such as weight-based dosing, age-specific considerations, and drug interactions.
* Provide expert pharmacotherapy consultation to healthcare providers, including physicians, nurses, and other members of the healthcare team, regarding medication selection, dosing, route of administration, and therapeutic monitoring for pediatric and neonatal patients.
* Research and provide accurate drug information to healthcare professionals, patients, and caregivers regarding pediatric and neonatal medications, including indications, contraindications, adverse effects, and alternative treatment options.
* Prepare and/or oversee the compounding, mixing, and dispensing of medications, including specialized formulations and intravenous (IV) admixtures, tailored to the unique needs of pediatric and neonatal patients.
* Implement medication safety practices and protocols specific to pediatric and neonatal populations, such as double-checking high-risk medications, verifying dosages, and ensuring proper labeling and storage of medications.
* Monitor and interpret laboratory data, such as drug levels and laboratory parameters, to assess the efficacy and safety of medications in pediatric and neonatal patients, adjusting dosages as necessary to achieve therapeutic goals.
* Participate in multidisciplinary rounds and patient care conferences in pediatric and neonatal units, providing input on medication-related issues, patient progress, and treatment plans.
* Educate parents and caregivers on medication administration techniques, dosage instructions, potential side effects, and the importance of adherence to medication regimens for pediatric and neonatal patients.
* Document and report adverse drug reactions, medication errors, and near misses according to institutional policies and regulatory requirements, and participate in root cause analysis and quality improvement initiatives.
* Collaborate with the medical staff and pharmacy and therapeutics (P&T) committee to develop and maintain a pediatric and neonatal drug formulary, ensuring the availability of safe and effective medications for use in these populations.
* Stay current with advances in pediatric and neonatal pharmacotherapy through continuing education, participation in research activities, and dissemination of knowledge to healthcare professionals, students, and the community.
* Participate in quality assurance activities, including medication use evaluations, performance improvement projects, and regulatory compliance audits, to optimize medication therapy outcomes and patient safety in pediatric and neonatal settings.

Requirements of the Clinical Generalist Pharmacist:

* Must possess a pharmacy degree from an ACPE-accredited institution.
* Completed a PGY1 pharmacy residency or have equivalent hospital pharmacy experience.
* Active Pharmacy licensure or eligibility in Indiana.
* Clinical Generalist experience with a high level of clinical practice skills.
* Enthusiasm for teaching/mentoring.
* Interpersonal skills to interact effectively with students, health professionals, and patients.
* Must be okay with mentoring, multidisciplinary rounding, and rotating through clinical/distribution services.

Requirements of the Pediatric/NICU Pharmacist:

* Must possess a pharmacy degree from an ACPE-accredited institution.
* Completed a PGY1 and PGY2 in pediatrics or equivalent pharmacy residency.
* Active Pharmacy licensure or eligibility in Indiana.
* NICU and Pediatric Pharmacy Experience with a high level of clinical practice skills.
* Enthusiasm for teaching/mentoring.
* Interpersonal skills to interact effectively with students, health professionals, and patients.
* Must be okay with mentoring, and multidisciplinary rounding.
* NICU-centered role, with coverage in peds on weekends, when needed.
* Must be okay with an 8 on/6 off schedule.
