The Hospice Licensed Practical Nurse (LPN) works closely with RN Case Manager(s) to provide holistic patient-centered care, quality care end of life. As part of a collaborative interdisciplinary team, the Hospice LPN provides compassionate supportive care to our Hospice patients, as well as their caregivers, and families.

Highlights for a Hospice Licensed Practical Nurse (LPN):

* One on one patient care visits in the home or care facility ALF/LTC
* Timely completion of required documentation, adhering to agency policy
* Attend and participate in weekly IDT meetings
* Manage comfort measures to ensure quality of life that is based upon the patient's goals
* Follow patient care plans, and orders
* Coordinate with RN Case Manager, attending providers and other IDT team members
* Engage in respectful communication (both verbal and written) with team members, patients, family members, and the community
* Adhere to agency wide policies
* Other duties as assigned

Qualifications for Homecare Registered Nurse (RN) Case Manager:

* Active NH LPN license
* Minimum 2-3 years direct care experience
* End of life or Hospice experience preferred
* Excellent communication skills necessary
* Proof of Flu vaccination
* Reliable transportation
* Valid driver's license
* Automobile insurance
* Physical exam within the last year
* Two-step TB test
