TheThe Homeless Case Worker will perform all aspects of case management with families staying in the hotel and families referred from DSS/211 for services including: engaging in client interviews to assess and understand the nature and extent of their challenges, which may include personal and family adjustments, potential foster care situations, financial struggles, employment issues, basic needs concerns, and physical or mental impairments. They will gather relevant information about the client's medical, psychological, and social factors, evaluate their capacities, and provide counseling and support to develop plans for meeting their needs and improving their social functioning. The Case Worker will actively refer clients to community resources and organizations, maintain accurate documentation and reporting for government agencies and funding sources, and ensure effective communication with supervisors and other stakeholders. Timely reporting of safety issues is also an essential responsibility. They will assist with the transportation of clients between the hotel and shelter and transporting clients moving from the Emergency Family Shelter to housing destination. They will help transport clients to appointments as needed and help transport clients to apartment viewings.

 

This position pays $21.00 Per hour


Education and/or Experience Required at Entry:

  • Associate Degree in Human Services or a related field.
  • Three years of experience working with vulnerable individuals.
  • Alternatively, a combination of education and relevant experience will be considered.
  • Candidates with lived experience are preferred and encouraged to apply.

Licenses/Certificate/Clearances:

  • Valid N.Y.S. Driver's License.
  • Registered and insured vehicle.
  • N.Y.S. Child Abuse Registry clearance.

Working Conditions and Environment/Physical Demands:

  • Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities.
  • Some locations may not meet Federal accessibility standards for disabled individuals.
