POSITION SUMMARY:

This is an advanced professional position that provides direct therapeutic services to identified mental health and substance abuse clients within the Homeless and Network Project Services Program as part of Gracepoint Adult Outpatient Community service. The Licensed Therapist will be responsible for maintaining patient care in accordance with program guidelines and external standards (Joint Commission, Medicaid, etc.). The therapist will provide clinical services specifically psychosocial assessments and treatment planning to the following populace: individual, family, and group therapy to children, adolescents, and adults.

QUALIFICATIONS:
  • Must be a current Licensed Therapist in the state of Florida (LMHC, LCSW, LMFT).
  • Bilingual (English-Spanish) is recommended due to the patient populace but not required.
  • Computer knowledge should include Microsoft applications (Word, Excel) and Electronic Medical Records experience
  • May provide clinical oversight in a multidisciplinary team.
  • Must have a Master's Degree in Health and Human Services or a similar related field of study.
  • Must be at least 21 years of age or older with a valid driver's license

FUNCTIONAL RESPONSIBILITIES INCLUDE:
  • Conduct thorough assessments and develop service, treatment, and discharge plans.
  • Provide individual, family, and group therapy sessions with adults, children, and families in an outpatient, acute care inpatient, or community setting.
  • Accurate and timely documentation of clinical and administrative services in accordance with program guidelines and external standards (Joint Commission, Medicaid, etc.).
  • Meets productivity requirements for the designated program through efficient workflow management.
  • Maintains cooperative relationships with other internal departments and external agencies and keeps supervisor informed at all times of all relevant client, program, and community issues
  • Understand the basic tenets of Co-Occurring disorders and the relationship between mental health and substance abuse. The therapist will also demonstrate knowledge of the age-specific needs of the clients treated in the program.
  • Adheres to all applicable federal, state, local, and company-maintained standards of compliance, ethics, and policies concerned with the administration and delivery of agency services.
  • Meets deadlines, requirements, and expectations as established by the supervisor, agency, and department.
  • Completes administrative functions and data entry timely and accurate.
  • Provides exceptional customer service in all communications and interactions with staff, and the general public.
  • Maintains confidentiality and remains calm under stress.
  • Assists staff with support in all areas of functional responsibility in a courteous, supportive manner.
  • Maintains and exhibits servant leadership qualities and exceeds customers' expectations
