Highlights
Community Physician Network is seeking a Physician to join our outpatient practice at our Family Medicine East Health Pavilion on the east side of Indianapolis, IN.
* Well-established and outpatient
* Shared call schedule with 24/7 RN triage
* East Health Pavilion also houses Family Medicine, Pediatrics, Dermatology, full lab services, X-ray and PT and rehab
* Patient Centered Medical Home (PCMH) practice model with pharmacist, dietician and care navigation nurse
* Electronic Medical Record system (Epic)
Licensure & Certifications
* Board certified/board eligible in Family Medicine
* Licensed in Indiana or eligible for medical licensure in Indiana
