Job Description

SBH Health System - St. Barnabas Hospital is a community hospital located in central Bronx, New York City. We have 400+ beds and is affiliated with Albert Einstein College of Medicine and CUNY School of Medicine. It is a multi-site health provider under SBH Health System, and is a designated Level 1 Trauma, Stroke, and AIDS Center.

SBH's General Surgery Department is expanding multiple programs and is seeking a full-time Advanced Clinical Coordinator (NP) to work in their Wound Care division. The ACC will be a key essential team member in the growth and development of the Wound Care program, to ensure patient quality care and upkeep of all requirements under DOH and Trauma 1 accreditation.

Responsibilities, but not limited to:

  • Make daily rounds with residents and attendings on inpatients (including documentation of wound status, treatments, plans, etc.)
  • Within scope of practice, debride wounds under supervision (mechanically as well as enzymatically), apply Unna boots, negative pressure devices, skin substitutes, etc. or similar devices - both at bedside and in outpatient settings
  • Respond to consults from other services, assessing and photographing wounds and documenting status
  • Coordinate with social service on hospital discharge, plans of care
  • Coordinate transfer of care to/from inpatient to outpatient environment
  • Coordinate with outside facilities on transfer of patients (including development of data-driven templates to facilitate care)
  • Coordinate medical care with primary providers that impacts wound management - including diabetologists, endocrinologists, orthotics, infectious disease, nutritional support -- as appropriate and indicated
  • Educate nurses and staff on aspects of wound prevention, staging documentation, treatment
  • Maintain a hospital wound care database
  • Facilitate the transfer of data from outpatient WoundDocs to hospital EHR (AllScripts and EPIC)
  • Participate in hyperbaric oxygen therapy of diabetic and other wounds
  • Help coordinate administrative functions of wound care, finances/payment and insurance issues
  • Participate in clinical research including prospective IRB approved studies
  • Participate in inpatient and outpatient "outreach programs" on wound issues

Requirements

Minimum Requirements:

  • Graduate of an accredited Nurse Practitioner Program
  • Active New York State Nurse Practitioner License
  • BLS, ACLS
