Job Description

PURPOSE OF POSITION

The Collaborative Drug Therapy Management (CDT M) Pharmacist partners with the Office of Ambulatory Care and collaborating physicians to support the mission of providing exceptional patient care across the Health System. They serve as a liaison between the patient, ambulatory, clinical and pharmacy teams, leading to efficient provision of drug therapy and positive patient outcomes. The CDTM Pharmacist provides consulting services to patients regarding the effective use of medications, importance of adherence, awareness of drug interactions, and may coordinate other preventive healthcare services such as immunizations. CDTM Pharmacist plays an essential role in disease state management, identifying ways to optimize medication therapy and ensuring an optimal patient experience.

SUMMARY OF DUTIES AND RESPONSIBILITIES

The responsibilities include, but are not limited to:
• Interviews patient to gather all relevant data, including but not limited to general health, activity status, medical history, medication history (including OTCs), and immunization history.
• Screens for drug allergy and drug interactions. Notes drugs or classes of drugs contraindicated in specific disease states.
• Assesses medication regimens and makes modifications so as to achieve desired therapeutic outcomes, providing direct pharmaceutical care in the outpatient ambulatory setting.
• Performs routine patient monitoring functions as necessary in drug therapy management, including collecting and reviewing patient history. Orders appropriate disease-state or drug specific laboratory and physical assessments including but not limited to vital signs, diabetic foot exams, hemoglobin Al C, and urine microalbumin (only if specifically authorized by the protocol and to the extent necessary to discharge their responsibilities under the law).
• Evaluates clinical laboratory tests related to the drug therapy management for the specific disease or disease state(s) specified within the Protocol.
• Adjusts the management of drug strength, frequency or route of administration and/or substitutes authorized drugs in accordance with the Drug Therapy Protocol with the Collaborating Provider.
• Performs comprehensive medication review and reconciliation and resolves medication-related problems in order to optimize drug therapy. Resolves discrepancies in patient therapy and follows-up on any outstanding issues or barriers.
• Documents modifications made to the patient drug therapy in patient's record and notifies other treating providers, as required. Documents and reports all interventions, adverse drug reactions (ADRs) and medication errors.
• Communicates any changes or adjustments in therapy to patients and caregivers in a clear and concise manner. Performs follow-up phone calls to patient, patient's caregiver and/or pharmacy to confirm compliance with filling prescriptions, compliance with medication regimen and understanding of instructions.
• Monitors the patient's response to therapy, including safety and effectiveness of medication.
• Presents requested drug information to patients, physicians, nurses, and other pharmacists, modifying terminology and approach based on target audience.
• Provides patient education on self-management and assists patients and providers with medication access issues to ensure understanding of the care plan.
• Prepares written notes in a consistent manner on all patients seen. Documents all progress notes and other relevant information in the electronic medical record.
• Works collaboratively with the care team to develop the patient care plan, promote adherence to treatment plan and encourage appropriate follow-up for optimal medical management.
• Supports and assists patient or caregiver to obtain prescriptions; confirms prescription insurance coverage, formulary status and ability of patient to obtain medications.
• Collaborates with patient assistance programs and finance team to eliminate or reduce financial barriers to therapy.
• Participates in educational programming for clinical staff, including, but not limited to, P&T meetings, noon conferences, grand rounds, in-services, and competency programs.
• Participates in the preceptorship of students during their clinical pharmacy education experiences; as applicable, participates in the supervision of pharmacy residents completing an ASHP-accredited pharmacy practice residency program.
• Maintains liaison relationships with medical and nursing staff, providing timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission Standards.
• Coordinates with ambulatory care team to standardize CDT M practices, creating goals for patients and implementing care plans.
• Establishes and measures patient outcomes against goals and objectives. Ensures results are defined and measurable, focusing on growth, improving outcomes, clinical effectiveness and patient safety.
• Maximizes the potential of clinical services to be offered by staff, including but not limited to immunizations and other opportunities consistent with the mission of the Health System under the direction of Senior Leadership.
• Makes recommendations to leadership regarding operational practices to drive favorable patient outcomes.
• Focuses on key performance indicators (KPI) and quality measures to evaluate and improve patient outcomes, as well as patient satisfaction.
• Ensures active communication between ambulatory care and pharmacy to establish strong relationships that are complimentary and consistent with organizational goals and strategic needs.
• Ensures that patients are properly allocated with appropriate resources in order to achieve positive outcomes, expectations and goals.
• Establishes and maintains working relationships within the organization in order to obtain feedback and content to continuously improve workflow and operations.
• Serves as an operational resource for internal team members to expedite problem identification and resolution of barriers to therapy, provision of patient-specific updates, and team interactions.

Minimum Qualifications
1. A valid New York State license and current registration to practice pharmacy; and
2. Must be certified to practice Collaborative Drug Therapy Management (CDTM) by the New York State
Education Department (NYSED).
