POSITION SUMMARY

The Start of Care (SOC) Clinician is assigned to provide the start of care service for new patients, participate in Admission / referral decision making and support the hand off from SOC to regional visit team members. Responsible for completion of OASIS assessment requirements for all new patients.

The SOC clinician launches the initial home-based patient encounter after pre-admission is completed. This position is responsible to provide assessment and care for the Home Health patient population requiring the use of advanced assessment, teaching and decision-making skills. This clinician is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary discussions take place regularly, and appropriate documentation is completed. Relevant knowledge and experience are consistently applied to new patient populations.

The SOC clinician provides support by completing necessary documentation including the OASIS to ensure proper care and services as well as appropriate reimbursement. During the SOC encounter there is responsibility for validating the patient appropriateness of home health and developing the home care plan in conjunction with the physician. The SOC clinician educates patients, families, caregivers, and community providers to safely perform care. Each SOC clinician initially belongs to a geographically organized clinical team. The SOC clinician also identifies performance improvement and home health standards of care initiatives and assists to design or implement programs to address needed changes.

The SOC clinician has knowledge of home care regulations and third-party reimbursement and how it impacts the delivery of services. This position may have on-call and/or weekend responsibilities and requires travel.

TECHNICAL RESPONSIBILITIES
  • Performs comprehensive assessments of physical, functional, psychosocial, and cognitive status of the home care patient utilizing interview observations and physical exam techniques.
  • Assesses the home environment for safety, infection control, and community resource needs.
  • Incorporates interdisciplinary data into the assessment of the home care patient.
  • Provides ongoing clinical assessment with the patient, family, and home environment to determine physiologic or psychosocial risk. Applies previous clinical experience and base of knowledge and adapts with the patient and family in a home setting.
  • Assesses for the presence of advanced directives and facilitates further action in obtaining information about or implementing advanced directives if indicated.
  • Plans with the patient, family and physician for care which is feasible within the physical, financial and emotional resources of the family.
  • Establishes individualized, measurable goals in consultation with the patient, family and other health care providers.
  • Anticipates home care needs and seeks a wide range of community resources to facilitate problem solving.
  • Utilizes home care standards and regulatory guidelines in developing an individualized care plan with each patient and family and participates in developing standards of care for patients in the home setting.
  • Maintains a working knowledge of community resources and refers patients and families appropriately.
  • Maintains technical skills according to agency standards, as measured by competency assessments during orientation and annually.
  • Implements safe, competent care with home care patients and families within the Home Health Agency's policies, procedures and standards of care.
  • Accurately follows established infection control and safety policies and procedures.
  • Directs the use of equipment and supplies in an efficient and cost-effective manner.
  • Utilizes resources to problem solve new or unfamiliar patient situations.
  • Documents components of the assessment process to reflect a comprehensive and integrated approach to nursing care.
  • Recognizes potential problems and sets priorities according to changes in the home care patient's condition.
  • In response to information indicating urgent and/or emergent risk to home care patients, notifies the physician immediately and notifies the supervisor or on-call administrator of the action taken.
  • Facilitates and participates in formal and informal interdisciplinary care conferences to address the needs of regarding complex patients.
  • Exhibits sound clinical judgment and decision-making skills in initiating patient care.
  • Identifies and adapts teaching materials for home care patients and families to optimize the learner success. Involves the home care patient and family in identification of individual requirements/perceptions of learning needs.
  • Develops, implements and evaluates individualized teaching programs using teach-back and return demonstration techniques to assess patient/family level of understanding and adjust teaching and plan of care accordingly.
  • Ensures that the home care patient and family demonstrate the knowledge and abilities regarding home care rights and responsibilities, diagnosis, health care status, treatment, skills, medication regime, advance directives, and adaptive behaviors gained as a result of teaching interventions.
  • Completes all SOC documentation requirements in a timely manner, including OASIS.
  • Acts as a resource person on clinical problems for other personnel caring with home care patients.
  • Fosters the professional development of self, clinicians and other home health staff.
  • Evaluates own performance in relation to home care standards.
  • Evaluates performance of peers as appropriate, in relation to home care standards of practice.
  • Participates in the development of program goals and evaluates progress toward goal achievement.
  • Participates in committees/meetings as delegated or appointed.
  • Maintains productivity according to agency standards after first 3 months of employment.
  • Provides care in a geographically logical manner by reading and updating directions to home in the medical record, and using map reading skills or GPS.
  • Maintains consistent availability throughout the workday.
  • Demonstrates leadership within a geographically organized work team, working with interdisciplinary professional and support personnel who provide care with patients, families, and caregivers in the community
  • Notifies patient/family/caregivers by 9 AM of the expected time for that day's visits.
  • Understands and attempts to meet the needs, wants, and expectations of internal and external customers.
  • Addresses concerns or complaints voiced by patients, families, caregivers, or other internal and external customers, and notifies supervisor/manager.
  • Able to recognize differences between values of self and others and administers care impartially regardless of the differences.
  • Identifies ethical conflicts and able to identify resources and options for addressing ethical issues and usual agency pattern, including involvement of the supervisor/manager, primary physician, and social services.
  • Involves the patient and family in the plan of care, incorporating their cultural, spiritual, and other belief systems.
  • Recognizes the importance of respecting individual patient and family choices regarding care and treatment options.
  • Assists leadership in identifying potential clinical practice issues and contributes to the development of specific plans to improve home care patient outcomes.
  • Demonstrates knowledge of research findings related to home health services by implementing current practice standards.
  • Incorporates changes in clinical practice to reflect new advances and ensure effectiveness of planned interventions.

Education:
  • Licensed based education degree or higher

Experience:
  • Minimum one-year relevant Home Health clinical experience required.
  • Recent, relevant experience with home health SOC encounters preferred.
  • Experience using home health EHR software applications such as Epic preferred.
  • Experience in teaching self-care skills to patients and families.

Miscellaneous:
  • Valid driver's license and current car insurance
  • CPR certification.
  • Knowledge regarding quality improvement and home health standards of care.
  • Knowledge regarding the impact of acute and chronic illness on the lives of patients and families in the community.
  • Functions with a high degree of independence.
  • Collaborates with other disciplines to provide care.
