CareStar, Inc. is currently seeking an Community Health Worker in the Clinical department to conduct program assessments using program and CareStar identified tools and process. The Community Health Worker is a home based position that works independently to manage a caseload as needed.

Flex staff as employed on an "as needed" basis. When his or her services are required by CareStar, Inc., the employee may work up to a maximum of 29 hours per week. Employees in this category are not eligible for benefits unless otherwise set forth in the Employee Benefit Eligibility Policy, but are eligible for mileage reimbursement pursuant to CareStar policy.

THE POSITION

CareStar is looking for motivated individuals who/to:
  • Active license as RN, LSW, or LISW is preferred.
  • 12 months experience in home and community based services within the past 5 years.
  • Knowledge of Medicare, Medicaid, home health care and Home and Community-Based waiver programs desirable.
  • Adheres to the CareStar Rule in performance of job responsibilities.
  • Understands and complies with CareStar Policies and Procedures.
  • Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

THE LOCATION

This position is open in Scioto County and is responsible for covering the surrounding counties.
