**Position Purpose:**

As a Lead Personal Care Assistant/Worker at Trinity Health PACE, you will play a vital role in delivering compassionate care and support to our participants. You will assist with personal care needs, support health management, and collaborate with team members to maintain a high standard of service. This role also involves aiding the department manager with scheduling and guiding new colleagues.

**What You Will Do:**

+ Provide personal care assistance, including toileting, grooming, and mobility support.

+ Assist with nursing care and ensure participant safety by identifying hazards.

+ Engage in activities such as conversation, recreation, and meal serving.

+ Support fellow staff and participate in interdisciplinary team assessments.

+ Maintain accurate documentation of services and participant progress.

+ Participate in ongoing training and adhere to policies, regulations, and ethical standards.

**Minimum Qualifications:**

+ High school diploma or equivalent; CNA certification preferred.

+ At least one year of experience with frail or elderly populations, or willingness to undergo training.

+ Proficiency with Microsoft Office and strong communication skills.

+ Ability to work independently, adapt to change, and demonstrate organizational agility.

+ Must meet state health standards and have a valid Employee Medical Statement.
