Job Description
Job Title: Licensed Practical Nurse

Job Specialty: Skilled Nursing Facility

Job Duration: 13 weeks (Temp to Perm)

Shift: Mid - 5x8 - 03:00 PM - 11:00 PM

Guaranteed Hours: 40 hours per week

Experience: At least 1 year of experience, preferably 2 years or more

License: Current LPN licensure or temporary license as a licensed practical nurse petitioner required

Certifications: Basic Life Support for Healthcare Provider (BLS HCP) from AHA required

Must-Have: Willingness to transition from temp to permanent after the contract; ability to work every other weekend; reliable attendance

Job Description:

- Perform nursing care and assist with planning and implementing care for patient comfort and well-being under the supervision of a registered nurse.
- Maintain a safe working environment, perform job responsibilities in a safe manner, and maintain clinical and professional competency.
- Participate in patient assessments and determine nursing care needs based on relevant data.
- Recognize and determine the need for immediate intervention.
- Document patient observations, treatments, patient response, and nursing care using the nursing process.
- Assist with the development/revision of the care plan by providing data and identifying potential goals and interventions for review by the RN.
- Communicate with appropriate clinical staff on the status of a patient if significant changes occur.
- Implement the identified care plan under RN supervision and ensure task completion.
- Maintain awareness of patient progress and goals, documenting in medical records.
- Organize workload and prioritize responsibilities to deliver quality patient care efficiently.
- Gain knowledge and competence that reflects current nursing practice and perform only competent nursing skills within legal boundaries and facility policies.
- Document assessments, procedures, patient/family education, and communicate information to healthcare team members timely.
- Provide and assess the effectiveness of patient and family education under RN direction within legal scope of practice.

Physical Requirements:

- Work requires walking, standing, sitting, lifting, reaching, bending, pushing, and pulling.
- Must be able to lift and support up to 35 pounds.
- Must have intact sense of sight, hearing, smell, and touch, finger dexterity, and critical thinking ability.
- Respond quickly to changes in patient and unit conditions.
