Summary of Position: This position is responsible for providing patients with assistance in meeting their healthcare needs including enrollment in ACA Health Insurance, preparing disability applications for patients through the SOAR program, providing education and completing paperwork for new uninsured patients, referring patients to Medicaid, and referring patients to other community resources based on their expressed needs. Additional duties include creating and maintaining relationships with community agencies serving the organization's patient population through community outreach and engagement.

Performance Requirements: The following qualifications are the minimum necessary to adequately perform this job. However, any equivalent combination of experience, education, and training which provides the necessary knowledge, skills, and abilities would be acceptable, subject to any legal and/or regulatory requirements; Must be able to sit, stand and walk for long periods of time; Ability to read and understand the English language; Ability to effectively maintain confidentiality of records and communicate with all levels of personnel.

Experience: Experience in a medical office setting required.

Education: High School Diploma or GED certificate required; Bachelor's Degree preferred

Certification(s)/Licensure: SOAR Counselor, ACA Certified Application Counselor, Community Health Worker Certification, HMIS Certification/Training

Additional Required Skills: Knowledge of medical office software and comfort in using computers in daily work activity. Must have excellent verbal communication skills. Excellent organizational skills. Must be able to problem solve and have critical thinking skills. Understand and maintain patient confidentiality

Key Responsibilities
  • Provide excellent customer service to all constituencies - community members, patients, fellow employees, community agency staff.
  • Utilizing motivational interviewing techniques, ascertain individuals' current needs and resources.
  • Participate in Weekly Continuum of Care (COC) Case Conferencing Calls
  • Participate in COC Gaston County Work Group Meetings
  • Monitoring/Working the Gaston County Laundry & Shower Units 1 day per week-Additional day if schedule permits, or for scheduled Outreach Event.
  • Support the HNG Outreach Mobile Unit.
  • Networking with Coc Agencies
  • Based on information provided by the individual served, provide education about requirements for programs as well as program enrollment and/or referral.
  • In both internal and external interactions, disability, ACA insurance, HMIS Certification, FQHC, and community agency enrollment and program guidelines, policies, and procedures.
  • Participate in QI initiatives to improve processes and minimize errors and variation.
  • Work well with all appropriate staff to maintain efficient workflow.
  • Utilize data systems to update and communicate with staff in order to receive appropriate referrals and expedite patient processing
  • Adhere to all reporting, training, and certification requirements of SOAR, ACA, FQHC, and other community programs.
  • Document activities in The Connector, and other means as requested by team leads and/or program manager.
  • Maintain positive relationships with community members, healthcare providers and community partners.
  • Develop and maintain positive relationships with community members and agencies through community outreach and engagement.
  • Physical demands include but not limited to setting up tables, chairs, booths and IT equipment at off-site enrollment events.
