Description
The role of the PACE Clinic LPN (or RN) report to the clinic primary care providers. This position supplements the nursing care needs of participants as established in the Interdisciplinary Team (IDT) plan of care. This position assists the primary care providers in the collection of specific assessment data that is used by the IDT in the planning of care needs. This position performs acts, tasks or functions under the direction of the RNs, physician, or nurse practitioner, and acts only within the scope of his/her authority to practice. This position will spend 100 % of the time in all areas of the center throughout the workday. Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds. Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling. Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment.
Responsibilities
* Communicates with the participant, the family and all members of the interdisciplinary team in implementing the participants' plan of care.
* Assists with data collection for routine as well as episodic visits to the clinic; the LPN communicates his/her findings to the medical provider or the team for evaluating or modifying the plan of care.
* Prepares and administers medications as ordered by the physician
* Ensures medication is administered in a safe, accurate, and timely manner
* Demonstrates knowledge of the medications administered, including actions and side effects, and instructs the participant/family in safe administration of medications at home
* Signs, dates, and performs all charting/documentation of medications; the LPN reports and follows up on any medication errors in accordance with the policies and procedures of LIFE PACE; the LPN participates in the collection of information and maintains accurate records of participant functional status and care given.
* Advocates to others on behalf of the participant and demonstrates accountability in resolving participant concerns or issues
* Understands, complies with and promotes the Participant Bill of Rights and assesses and works toward achieving high levels of participant satisfaction.
* Performs phlebotomy functions when necessary
* Performs treatment(s) per policies and procedures
* Implements the orders written by primary care and nursing orders when necessary
* Educates participants and family regarding medications and treatment instructions and ensures understanding of given information
* Demonstrates a thorough understanding of HIPAA/HITECH laws and the related policies and procedures within the agency.
* Attends and successfully completes mandatory PACE training annually.

Qualifications
* Education: Graduated from accredited LPN nursing program
* Licensure and Certifications: Arkansas LPN license, Drivers License and auto insurance, CPR
* Experience: Minimum of one (1) years of documented experience working with a frail or elderly population
