Job Summary: The Coordinator-RN VAD Program is a highly experienced nurse specializing in cardiac Ventricular Assisted Device (VAD) and Destination Therapy (DT) and related aspects of care for critical ill patients in heart failure with multiple complicated comorbidities. Serves in a patient-centered care model of practice in a large Magnet Recognized academic medical center with level one trauma designation, ST-Elevation Myocardial Infarction (STEMI) receiving center designation, chest pain center accreditation, Advanced Certification in Ventricular Assist Device (VAD) and Comprehensive Stroke Center Accreditation. Understands the uniqueness of cardiac diseases and the sensitivity of communicating with patients facing the unknown of a serious critical illness. Intervenes as a patient advocate as appropriate. Evaluates VAD and DT program candidates and coordinates access to care based on established criteria. Applies specialized VAD and DT case management and care coordination methodologies and practices to provide a seamless multidisciplinary approach to care. Ensures effective utilization of resources, optimal care coordination and the provision of cutting-edge quality care for this specific patient population. Provides VAD education to first responders and the emergency departments in the community where the patient resides. Makes home visits as needed to evaluate electrical status and ability to support a VAD. Works collaboratively with the director, administrative director, medical director and other key individuals to maintain Advanced Certification in Ventricular Assist Device (VAD) program accreditation issued by The Joint Commission. Maintains electronic VAD and DT database(s) as required for program accreditation and tracking performance metrics. Builds meaningful reports and presents data to leadership team and quality committees for a systematic and logical approach to the decision making process to improve patient safety, clinical outcomes, financial performance and customer satisfaction. Supports clinical and translational research activities and participates in Institutional Review Board (IRB) approved research that improves outcomes and quality of life for critical ill patients in heart failure with multiple complicated comorbidities. Utilizes interpersonal skills to maximize excellence in customer service, safe patient care and professionalism. Demonstrates expertise in service recovery methodologies and capably resolves issues from dissatisfied customers. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed.

Education and Experience: Bachelor's Degree in nursing required. Master's Degree in nursing preferred. Minimum of two years' experience caring for patients with cardiac diseases required. Cardiac intensive care nursing experience preferred.

Knowledge and Skills: Substantial knowledge of cardiovascular diseases and treatment modalities for critically ill patients with progressive heart failure requiring mechanical circulatory support device required. Knowledge of state and federal healthcare regulations and accreditation, finance, performance improvement, quality, patient safety, evidenced-based clinical practice standards, medical staff structure, legal aspects of care, and nursing scope of practice required. Ability to apply theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in cardiovascular disease management. Ability to work effectively with a wide variety of staff. Knowledge and application of adult learning principles. Able to read; write and speak with professional quality; use computer and software programs necessary to the position (e.g., Word, Excel, Power Point, Access); operate/troubleshoot basic office equipment required for the position. Able to relate and communicate positively, effectively, and professionally with others; provide leadership; be assertive and consistent in enforcing policies ensuring compliance with regulatory standards, rules and laws; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English (and Spanish preferred) in person, in writing, and on the telephone; think critically; work independently; perform basic math and statistical functions; manage multiple assignments; compose written material; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone, and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position.

Licensures and Certifications: Active California Registered Nurse (RN) license required. Basic life Support (BLS) certification and Advanced Cardiac Life Support (ACLS) certification issued by the American Heart Association required.
