Responsibilities

Job Summary

Duties to include but not limited to: the assessment/evaluation of patient withdrawal followed by the execution of appropriate medication detox protocols; administration of prescribed medications and patient education.

* Provide patients with a safe medically managed detox by assessing signs and symptoms of withdrawal. Monitors the effects of medications administered and documents accurately and appropriately.
* Reports and documents medication errors, pharmacy errors, packaging errors, medication count concerns, etc.
* Ensures that ongoing assessments are conducted as determined by the patient's condition using COWS & CIWAS and re-prioritizes care accordingly.
* Maintains neat and accurate narcotic control inventories and all documentation related to the administration of medications.
* Promotes patient's independence by establishing patient care goals, teaching patient to understand condition, medications, and self-care requirements, answers patient questions.
* Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following philosophies and standards of care set by State Board of Nursing, and other governing agency regulations.
* Resolves patient problems and needs by utilizing multidisciplinary team strategies, including de-escalation techniques.
* Documents patient care services by charting in EMR.
* Maintains continuity among teams by documenting and communicating actions, irregularities, and continuing needs.
* Contributes to team effort by accomplishing related results as needed.
* Participates in the onboarding of new nurses for medication administration.
* At end of shift communicate to incoming LPN pertinent data regarding patient activity and conducts medication count.
* Treats patients, and colleagues with dignity and respect; honors each person's right to privacy/maintains very strict patient and colleague confidentiality; respects all individual differences; supports cultural diversity.
* Knows role and responds appropriately to all codes related to emergency situations.
* Consistently arrives to work on time and adheres to attendance policies.
* Other duties, as needed.
