$2,500 - $7,500 Sign On Bonus*# SUMMARY:# The LPN, Clinic Case Manager, through patient assessment and engagement, chart review and collaboration with the clinic healthcare team, develops an appropriate care plan, making recommendations for alternative levels of care, identifying cost effective care options and facilitates appropriate clinical follow up. This work is done through patient assessment and engagement, chart review and collaboration with the healthcare team.# The LPN, Clinic Case Manager monitors patient progress throughout hospitalization or office visits, modifying plans as needed and continually assessing medical necessity. The LPN, Clinic Case Manager provides information to post hospital vendors as needed to ensure ongoing recovery for the patient. JOB REQUIREMENTS Education: Graduate of an accredited school of nursing. LPN Required.#### Licensure: Current Arkansas or Compact State Licensed Practical#Nurse license in good standing. Experience: Willing to train recent LPN graduate.# Previous Clinic experience desired. Certifications: BLS required. Other: Ability to read, write and communicate effectively. Basic computer knowledge required. Ability to regularly stand and walk, occasionally bend, squat and twist and occasionally requires lifting of 50 pounds or more of equipment using proper body mechanics.# Able to push in excess of 150 pound wheelchair with patient and occasionally lifts patients with assist 125 pounds or more with proper body mechanics. Spends eight hours or more in front of computer, monitor or similar screen utilizing keyboard and/or mouse. Must be able to respond quickly in emergency situations of CPR and obtaining equipment. Must be able to handle highly stressful situations with efficiency and composure. JOB REQUIREMENTS
