Job Overview:

The VAD coordinator is responsible for the coordination of inpatient and outpatient care for patients with a ventricular assist device. These patients are candidates for advanced heart failure therapy using VAD support with the intent of either bridge to transplant or destination therapy. The VAD coordinator is a motivated, self-directed, and engaging individual who possess knowledge and expertise in the practice of mechanical circulatory support (MCS). Directs the VAD program operation, develops strategy, and oversees financial management for optimal outcomes.

Job Requirements:

Bachelor's Degree in Nursing

Registered Nurse

Understanding in the care of Adult Cardiology and Cardiothoracic critical care patients with acute care Experience (critical care) required

3-4 years experience Clinical Nursing

Job Responsibilities:

Educates patients/families/nursing staff in VAD evaluation process, and clinical management of patients through inpatient/outpatient phases. Provides education and coordination, including risks and benefits, to VAD candidates and post-implant patients throughout the entire patient experience, and the ongoing development of patient and caregiver education materials. Independently assesses and analyzes clinical findings and is a resource to patients. Assists with ongoing troubleshooting. Collaborates with clinical pathway team and appropriate leadership to improve quality of care. Evaluates competency of clinical staff/patients/family members/physicians/community health resources.

Acts as a consultant within TriHealth and the VAD community. Identifies competency/learning needs and collaborates with department director, clinical coordinators and staff in designing orientation and on-going educational activities related to care of VAD patients. Develops educational materials and provides classes for nursing staff and VAD team members. Develops and integrates new and existing practices into the VAD Program Policy and Procedure Manual. Reviews and updates guidelines annually and PRN.

Provides education and support to community health care professionals who may encounter VAD patients, including home health nurses, referring physicians, and first responders (EMTs, paramedics, local emergency departments). Provides VAD program information during negotiation of managed care VAD contracts. Assures that all VAD patients have an identified PCP and that these MD's receive correspondence and educational material on their VAD patients. Develops resource materials for referring physician.

Develops strategies to improve communication between the VAD program and outside providers.

Documents appropriate information in patient's medical record. Responsible for maintaining the INTERMACS database. Prepares statistical reports and analyses data and reports on progress and adverse trends. Maintains a patient management system and collects and documents required regulatory and program specific data.

Works with the Hatton Research Institute to involve patients in any appropriate research trials.
