Join our dedicated team as a Licensed Practical Nurse (LPN) in our intimate 9-bed Inpatient Rehabilitation Unit at UPMC, where you'll make a meaningful impact on patients' recovery journeys

**Responsibilities:**

+ Collaborate with the healthcare team, exercising sound nursing judgment in patient care.

+ Participate in planning, implementing, and evaluating nursing care using focused assessments.

+ Communicate effectively with licensed professionals and healthcare team members, especially when care needs exceed LPN scope.

+ Follow established UPMC policies and procedures, including LPN Practice Guidelines and State Practice Act.

+ Create individualized care plans and daily goals, involving patients as partners in their care.

+ Provide comprehensive patient documentation to enhance communication among caregivers.

+ Foster a compassionate patient experience by building healing relationships with patients, families, and colleagues.

+ Deliver thorough teaching to patients and families to ensure effective transitions in care.

+ Support the development of new LPNs and patient care staff while seeking continuous professional improvement.

+ Identify and participate in quality improvement initiatives to enhance patient safety and satisfaction

+ Graduate from a State Board-approved Licensed Practical Nursing Program.

+ Minimum of 1 year of nursing experience.

+ Successful completion of an IV therapy curriculum required within 4 months of hire (proof needed for graduates before 2012).

+ Strong collaboration skills to work effectively with the healthcare team, patients, and families.

+ Excellent interpersonal skills to maintain positive relationships with colleagues and patients.

+ Physical stamina for patient mobility and ability to use specialized equipment.

+ Proficient in using electronic health records.

Licensure and Certifications:

+ Must maintain state licensure or be part of a licensure compact agreement.

+ Complete one annual contribution to improve patient care or maintain a UPMC-approved certification.

+ Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

+ Licensed Practical Nurse (LPN)

+ NIH Stroke Scale (NIH)

+ Act 34
