Performance Improvement Coord/Ed Spec-Home Care

Position Summary: The Performance Improvement Coordinator/Education Specialist for Home Care works with the Director of Home Care and Rehab to coordinate department activities. Supports the director and manager#s efforts to track and trend patient outcomes. Infection control program and records management. Coordinates with Clinical Managers, for orientation, training, and development of Agency staff. Directly supports the Quality Assurance program to meet Agency standards, licensure regulations and federal and state regulatory initiatives. Education: Bachelor of Science degree in Nursing. Master#s degree in health care or nursing preferred. Licensure: RI state RN licensed. MA State RN license required within 60 days from date of hire. Active Driver#s license. BLS required. BLS Instructor within 12 months of hire. Experience: Four (4) years full-time clinical nursing experience, at least two of which were in a home health agency. Formal and informal experience teaching, Wound Care certification. Ostomy certification (preferred). Knowledge of Community: Health and Home Care Principles. Knowledge of Agency Policy and Procedures. In-depth knowledge of Medicare criteria and ability to apply and remain in compliance with accreditation and licensure regulations. Excellent interpersonal relations skills, oral and written communication skills, and organizational skills. Excellent problem-solving skills. Ability to educate and evaluate staff field performance and provide education as needed. Ability to assess and create care plans/update care plans for ostomy and wound care patients. Possess adequate transportation to carry out job requirements. Must show proof of satisfactory automobile insurance with minimum coverage per Agency policy for bodily injury and property damage when using automobile for transportation. Position: Full-time/Non-Union Hours: Monday-Friday from 8 am # 4:30 pm # Essential Responsibilities: # Participates in their own professional growth and development. # Continuously carries out the CCHP Mission to attain excellence in the provision of healthcare services by consistently fulfilling the commitment to quality, and by compassionately and skillfully delivering continuously improving total client centered service. # Knowledge of agency policies and procedures. # Seeks appropriate supervision. # Communicates with the interdisciplinary team. # Active listening skills and communication skills with field staff, director and manager(s). # Ability to define problems collects data, establish facts and draw conclusions # Commitment to teamwork and team # Problem solves in a solution orientated manner. # Participates actively in self-appraisal and the formation of a plan of self-development. # Demonstrates knowledge of agency software programs. # Responsible for staff training/education in the basics of Home Health, Wound and Ostomy care, Infection Control practice and procedures. # Evaluates clinical staff and assesses for compliance with policies and procedures. # Performs ongoing orientation and education for clinical staff and participates in ongoing staff development programs. # Adheres to the Agency Corporate Compliance Plan and HIPAA Privacy Plan per policy # Completes clinical audits in compliance with RI State, CMS and Joint Commission regulations. # Weekly visits all wound assess wound measurements and treatments as ordered by the Health Care Provider (HCP). # Bi-weekly assessments and treatment # Develops treatment plans for all wound patients and ostomy patients and communicates with HCP and Wound Centers. # Home visits to assess education and performance improvement. # Develops and updates to patient centered care plan. # Responsible for Agency#s infection control program: # Participates in Quality Improvement initiatives and functions: a. Identifies both positive and negative trends in OASIS, documentation and care planning. b. Participates in individual clinician education through one-on-one review and discussion. c. Participates in all Quality Improvement activities and functions as needed. # Conducts quarterly infection control audits of patient records. # Tracks and trends the results of the infection control program. # Evaluates the infection control program against national and regional standards and makes recommendations for changes to program and staff education # Maintains communication with wound and ostomy clinics. # Communicates with the Health Care Provider for coordination of care and treatment plan. # Conduct annual skills competency fair annually and as needed. # Establishes and maintains systematic processes for review of educational material to ensure content aligns with PQI, Best Practices guidelines, and Policies/Procedures # Assists in the identification of knowledge deficits in Agency clinical and management staff and assists with the quarterly education plan to address those knowledge gaps. # Maintains and orders company supplies. # Works with medical companies on effective wound and ostomy protocols. # Participates in QAPI and Professional Advisory meetings and presents clinical data. # Completes full orientation of new employees, including documentation, OASIS training and field visits. # Other activities and other duties as assigned. Education: BSN in nursing preferred. Certification in Wound is required/ Ostomy certification is preferred.
