Position Summary Under Registered Nurse supervision, the Licensed Practical Nurse demonstrates thorough knowledge of therapeutic patient care, patient advocacy, team dynamics, hospital systems, and serves as a resource for support staff.# Primary focus is to support admission, discharge, and transfer processes according to policy and procedures.# Provides direct patient care and supports department operations as indicated by leadership. # Minimum Requirements Education######### Graduate of an accredited / approved Practical Nursing Program # Experience######## Two years# experience # License/Registration/Certifications############# Current LPN licensure in the State of South Carolina or Compact State BLS # may secure certification post-hire # Preferred Requirements # Preferred Education##### N/A Preferred Experience#### N/A Preferred License/Registration/Certifications############# N/A # Core Job Responsibilities Successfully completes hospital and unit-based orientation programs Responsible for demonstrating competencies for equipment and procedures Under the direction of an RN, assesses assigned patients# health care data, medical history, admitting diagnosis and physician#s orders, evaluating diagnostic data, interviewing patient and family to assist in planning and to determine patient needs Administers medication and treatments as prescribed Provides, documents, and supports the education of patient and family Provides and documents accurate information to patient and family per the after-visit summary and discharge orders Supports the transfer process Communicates accordingly with the health care team advising any areas of concern or changes in condition Assures continuity of care through effective decision making resulting in quality patient care Communicates pertinent information with health care team.# Documents all aspects of patient care by following established charting procedure to show patient#s status and progress or any change in condition Collaborates with patients, families, and other members of the health care team to determine patient priorities and treatments Initiates and facilitates interdisciplinary collaboration to enhance outcome Active participation in huddles and communication regarding patient status and readiness for discharge Seeks necessary assistance in completing nursing interventions to assure positive patient outcomes Participates in purposeful rounding, bedside shift report, huddles, and updating Care Board information Ensures compliance with all clinical expectations regarding regulatory requirements Provides clinical guidance to students and ancillary staff Complies with established policies and procedures Formulates a plan for professional growth Adheres to proper chain of command Other duties as assigned
