Position Purpose The Clinical Pharmacist_Amb Care provides evidence based pharmaceutical care services to include: # Pharmacotherapy consults in the form of comprehensive medication management # Disease management support # Medication reconciliation # Assessment of health and pharmacotherapy issues # Recommendations to support the patient#s longitudinal plan of care # # # Nature and Scope As a part of the healthcare team: # Provides face to face, telephonic, or virtual visit pharmaceutical care services to patients and works to promote appropriate, safe, and cost-effective drug therapy recommendations to improve the quality of our patient#s lives. # Assists patients and physicians with medication profiles and understands the patient formulary coverage to ensure that we find the best medication for the prescribed plan of care. # Performs medication monitoring, disease state management counseling, and collaborative drug therapy management. Certification(s): Healthcare BLS within the first 90 days of employment is required for all locations. Preferred: Board Certified Ambulatory Care Pharmacist (BCACP) or Board-Certified Pharmacotherapy Specialist (BCPS) Computer / Typing: Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel, and Word and have the ability to use the computer to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc. # #

Position Purpose

Nature and Scope
