Pharmacy Medication Navigator/Reconciliation Technician (PMRT) works under the supervision and instruction of a licensed pharmacist, performs procedures and duties relating to the service's Scope of Care. PMRT plays an integral role in improving patient care outcomes as the patient transfers through the various interfaces of institutional care at Beaufort Memorial Hospital (BMH). PMRT obtains a Best Possible Medication History (BPMH) through interviewing the patient/patient's family, contacting community pharmacies, contacting providers, reviewing previous hospital records and outside electronic systems and any paper records. PMRT facilitates discharge medication reconciliation by assisting in completing the Best Possible Medication Discharge Plan (BPMDP). PMRT collaborates with the BMH providers, inpatient pharmacists, and outpatient/community pharmacists on determination of the patient's medication needs for uninsured and indigent patients. PMRT helps promote a smooth discharge medication process, enhance the overall health of patients and decreasing readmissions.
