Position Purpose The Diabetes Educator is responsible for coordinating and conducting individual and group patient assessment and education for both diabetes and other/like chronic health conditions. Works in collaboration with physician(s) in the clinic setting to evaluate patient needs and individualize patient education. Patient education may include but is not limited to diabetes self-management (blood glucose monitoring/interpretation, carbohydrate counting, medication action/ usage and acute and chronic complications) diabetes and pregnancy, support groups, data analysis and management of patient blood glucose levels with physician orders. Nature and Scope Demonstrates the knowledge and skills necessary to provide care, based upon physical, motor/sensory, psychosocial, and safety appropriate to the age of the patient served. Current CPR certification required. # Computer / Typing: Must possess, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc. EMR trained and competent.

Minimum Qualifications

Name Description

Education:

Must have working-level knowledge of the English language, including reading, writing, and speaking English.

Experience:

Two years of applicable and recent clinical experience.

License(s):

State of NV Registered Dietitian license required

Certification(s):

Certified Diabetes Educator required within 90 days of employment. Current CPR certification required.

Computer / Typing:
