Summary:

The Pharmacy Medication History Technician demonstrates excellent customer service performance in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission, and Values of Covenant Healthcare and the commitment to Extraordinary Care for Every Generation. Provides technical assistance in the documentation and clarification of medication and allergy histories for patients admitted to the hospital. This information will be used by the physicians and other health care providers to determine treatment plans and will provide the basis for medication reconciliation of orders during and following the hospital stay. The Pharmacy Medication History Technician must communicate effectively and courteously with patients and health care providers, using interview skills, listening techniques, medication knowledge, and available resources to develop a complete and accurate electronic list of medications and allergies for each patient. Pharmacy Medication History Technicians will also be responsible for performing other specialty functions, including but not limited to, workload evaluation, position justification, continuing quality improvement, or other assignments in support of inpatient and outpatient pharmacy operations.

Responsibilities:

Participates in cost containment efforts through efficient use of supplies, services, and time management.

Remains flexible in cooperation with staffing patterns and scheduling needs for the provision of Pharmacy services and patient care needs.

Follows all Covenant Healthcare policies in regards to attendance, punctuality, and break times.

Communicates with customers in a professional and courteous manner, maintaining the highest level of confidentiality.

Perform medication history assessments for new admissions to the hospital, providing complete information regarding each medication (i.e., drug, dose, route, frequency, indication, and last dose taken-date/time).

Verify patient allergy information and ensure complete documentation (i.e., substance, allergy category, reaction description).

Obtain Medication Histories and allergy information for new admissions to the hospital in a timely manner, utilizing all possible resources (i.e., communication with patient, medication history lists, medications brought in by patient, physicians (family or current attending), nursing, retail pharmacies, or other medical facilities where the patient may be transferring from.

EDUCATION/EXPERIENCE

High school diploma is required, Associate's degree preferred.

Current Pharmacy Technician Certification from a Nationally recognized organization.

If exceptions are made, the technician must obtain certification no later than six months after employment or employment will be terminated.

Current Pharmacy Technician Licensure in the State of Michigan.

Previous experience as a retail Outpatient Pharmacy Technician required, with experience in transcription duties desired.

KNOWLEDGE/SKILLS/ABILITIES

Math and pharmaceutical proficiency must be demonstrated by testing and observation.

Must be able to work well with the public and able to be tactful in often hectic and stressful situations.

Includes proper phone etiquette.

Outstanding and effective communication skills.

Must be able to learn the operation of new pharmacy equipment effectively.

Computer competency.

Proficiency in Microsoft Word, Excel or equivalent.

Sound judgment in handling confidential or sensitive issues and material.

Ability to work with accuracy and meet established deadlines.

Ability to work independently as needed.

Ability to follow written and verbal instructions

WORKING CONDITIONS/PHYSICAL DEMANDS

Ability to maintain punctual attendance consistent with the ADA, FMLA, and other federal, state, and local standards.

Constant feeling and talking.

Frequent lifting up to 10 lbs.

Frequent standing, walking, pushing, feeling, near vision, midrange vision, depth perception, visual accommodation, color vision and field of vision.

Occasional lifting up to 50 lbs.

Occasional sitting, lifting, carrying, pulling, climbing, balancing, stooping, kneeling, crouching, squatting, twisting, reaching, handling and far vision.
