Pediatric Registered Nurse Coordinator/Case Manager

The Nurse Coordinator reports directly to the Director regarding the ongoing operations of his/her caseload. This includes the provision of case management services to eligible recipients and the administrative processing of referrals from the Illinois Department of Human Services, the Illinois Department of Public Health (IDPH) and other referral sources. Additionally, the Nurse Coordinator is responsible for interfacing with staff of various agencies providing referral, support, medical, and generic services to the clients and working in & meeting the additional requirements of the High Risk Infant Follow-up (HRIF) Program as needed.
POSITION QUALIFICATIONS:
Bachelors degree in Nursing (with an active Illinois RN license) and a minimum of two years of experience with pregnant women, children 0 to 1 year of age and their families, or experience with community health, maternal and child health care.
This position requires an individual who is capable of interfacing with staff of various agencies providing referral, medical, support, and generic services to eligible recipients. A thorough knowledge of the Department of Human Services, Department of Public Health, prenatal care, infant/child development, community support and generic support is required.
Job Responsibilities:
Conduct preliminary reviews of referrals, complete intakes, and perform risk assessments (711 & 712).
Assign cases and coordinate with IDHS and other public agencies, maintaining effective interagency relationships.
Visit service sites and clients' homes as required, and discuss service referrals with clients and their families.
Identify, negotiate, and arrange for service delivery; maintain records and documentation for all cases.
Provide training, technical assistance, and participate in team meetings, special projects, and program evaluations.
Ensure timely entry of client information, appointments, and service data into tracking systems (e.g., Cornerstone Scheduler).
Meet performance requirements for client interactions, developmental assessments, depression screenings, and reporting.
Manage client care plans, immunization education, well-baby visits, and referrals to PCP, WIC, family planning, etc.
Follow policies and procedures (ASQ, EPDS, FCM) and collaborate on audits, quality improvement plans, and case management staffing.
Provide supervisory support in the absence of Directors/Program Managers and assist Case Managers as needed.
Maintain flexibility in working hours and remain reachable by phone until 5:00 pm on weekdays; be on call as required.
