*Position Summary: *Provides psychosocial, preventative, and therapeutic support and health services to cases of assigned or referred patients. Coordinates with various providers across the patient’s health care team to ensure continuity of care and effective delivery of planned treatment. Educates patients and their families regarding the medical, socioeconomic, and mental health related resources available to them.
* *Shift Schedule: * Days, 8 hour shift
* *Department: * Behavioral Health Services
* *Location:* Sacaton, AZ

*Required Qualifications:*
* Master’s Degree in Social Work from a school accredited by the Council on Social Work Education (CSWE).
* Current and valid State of Arizona license as a Social Worker.
* 2 years of experience in Case Management required.
* Current and continuing CPR and AED training and certifications.
* AZ Department of Public Safety Fingerprint Clearance Card.
* State of AZ Driver’s License with no significant violations during the last 5 years.
* Demonstrates ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
